What is the ocean state clinical collaborative? • Established in 2004 • Statewide association of 21 Clinical Pharmacists • Representing 11 RI hospitals • One SE MA hospital • Share ideas and strategies for improving patient care throughout Rhode Island’s institutions
The Pharmacy Practice model initiative (ppmi) ASHP Pharmacy Practice Model Consensus – 2011 Multiyear effort organized by ASHP All patients should have a right to the care of a pharmacist Health-system pharmacists must be responsible and accountable for patients’ medication related outcomes Pharmacy department staff needs guidance on how to most effectively establish an optimal practice model Goal: “significantly advance the health and well-being of patients by developing and disseminating a futuristic practice model that supports the most effective use of pharmacists as direct patient care providers”

Why is this important? Vision: The Pharmacy Departments at Rhode Island hospitals will coordinate their efforts to offer the Rhode Island community a level of clinical pharmacy services that is both cost effective and improves clinical outcomes. Mission: To determine the current perception of pharmacy patient centered services at individual hospitals in Rhode Island and identify and prioritize areas where services could be improved.
What information will we be asking? • Anonymous, Demographic Questions • Individuals will not be identifiable. • Hospitals will only be subdivided by census size. • May choose not to respond • 1. Chose one title that most closely fits your role: • Staff Pharmacist (centralized) • Clinical Pharmacist (specialist/decentralized/outpatient/interdisciplinary) • Pharmacy Supervisor/ Director/Manager/Coordinator • IS Pharmacy Specialist 2. What this survey can accomplish • Goals: • Identify a framework of the basic clinical services that currently could be provided consistently at every RI hospital • Identify how the collaborative could support hospitals in developing enthusiasm for the new model and the tools to facilitate implementation of the recommendations • Promote and support educational resources for establishing these basic clinical services at every RI hospital
