  3. Incidentie (2) • Solitair • Enkelvoudig • Multipel

  4. Incidentie (3) • Gemiddelde overleving= 3-4 maanden

  5. Ontstaanswijze • Metastasen ontstaan meestal door hematogene verspreiding • Primaire hersentumoren metastaseren meestal naar de longen

  6. Symptomen t.g.v. • Directie druk op of beschadiging van hersenweefsel • Oedeem • Inklemming

  7. Soort symptomen (1) *Hoofdpijn ( 25-50%) *Misselijkheid ( <10%) *Braken ( < 10%) *Cognitieve en gedragsstoornissen ( 15-35%) *Epilepsie ( 10-25%) *Ataxie en loopstoornissen ( 10-33%) *Krachtsverlies ( 15-33%)

  8. Soort symptomen (2) *Afasie ( 9-15%) *Sufheid ( <10%) *Visusstoornissen ( <10%) *Duizeligheid ( <10%) *Sensibiliteitsstoornissen ( <10%)

  9. Inklemmen • Delen van de hersenen worden verplaatst onder de falx cerebri, het tentorium cerebelli of door het achterhoofdsgat Leidt tot uitval hersenstamfuncties

  10. Klachten bij inklemmen • Hoofdpijn • Misselijkheid • Toename neurologische uitval • Bewustzijnsdaling • Hypertensie • Verhoogde T • Vertraagde hartslag

  11. Aard en ernst symptomen • Plaats hersenmeta • Grootte meta • Aantal meta’s • Hoeveelheid omgevend hersenoedeem

  12. Acute presentatie • Epilepsie • Infarcering • Bloeding in een metastase • Inklemming

  13. Behandeling • Bij symptomatische hersenmetastasen: Dexamethason 1dd 4-16mg

  14. Enkelvoudige/solitaire hersenmetastasen • Resectie • Stereotactische radiotherapie

  15. Multiple hersenmetastasen • <4: stereotactische radiotherapie • >4: totale schedelbestraling

  16. Bij chemo- of hormoongevoelige tumoren: deze behandeling overwegen
