This session provides information on the reporting process for health home data in NY State, including the population to report, data flow, submission process, and reporting periods. Questions and feedback are encouraged.

HH-CMART – Reporting Population • HH-CMART data will be captured for all Medicaid clients who are assigned or engaged in a Health home for the reporting period. • This includes all clients in the converting TCM programs. Clients in the TCM are considered engaged in the Health Home as of the effective data of the SPA for each Phase (January 2012 for Phase 1, April 2012 for Phase 2, and July 2012 for Phase 3). • This includes new clients who are assigned to the Health Home, as either outreach or engagement, during the reporting period. • This does NOT include information for clients who are not Health Home members, but who are receiving care management services from the care management organization.
Care managers will provide needed information about services provided to the Health Home Health Home enters or imports data for all members assigned to the Health Home into the tool and submits HH-CMART to DOH HH-CMART Data flow Health Home collects data from all care management staff involved with its members • Data needed for the HH-CMART will be gathered by the care management providers (either in the HH-CMART tool or in a template file) using the specifications and response options for each element. • For any care management provider managing members for more than one Health Home, multiple HH-CMART files can be used. Make a copy of the HH CMART Tool to use for each of the health homes separately prior to entering any data. You should not use the same HH CMART Tool for entering more than one health home’s data. • At the end of the reporting period, the care management provider securely transmits a file to each Health Home containing the data for that Health Homes members. • Health Homes gather the care management data from their care management providers for their members and aggregate all of the data files into the Health Home’s file used for HH-CMART. The Health Home’s file is imported into the HH-CMART for the Health Home for that reporting period. • The HH-CMART file for the Health Home is sent to SDOH via the Health Commerce System.
HH-CMART File Submission Process Health Homes will collect data from care management providers for the reporting period and import or enter data into a copy of the HH-CMART. Health Homes will review the reports in the tool, correct errors as needed. The completed tool should be saved as the quarter’s file (ie. 4Q2012.mdb) and the data can be exported out to be used as the template for importing the next quarter. Completed files are sent through the Health Commerce System (HCS) using the ‘Secure File Transfer Application’ from the Applications tab. Name the file with the Health Home name and upload the file (ie. CapitalHealth.mdb). Send the file to ‘Laura Morris’.
Reporting Periods and File Submission Dates For each file submission, use a copy of the original version of the HH-CMART
