What’s required in an approved training agreement? • Student’s personal information: name, address, telephone, birth date, emergency contact • School name, address, telephone (2x), contact person • Employer name, address, telephone, contact person • List of student, employer, and school responsibilities • Date of appropriate safety instruction (employer or school provided) • Beginning & end dates of agreement • Daily hours to be worked, including beginning & end times • Verification of workman’s compensation & general liability insurance • Signatures of: student, parent/guardian, employer, coordinator, & principal or designee www.rpmoll.com
What’s required in an approved training agreement? • The following items are required, but will not be reviewed by the auditor • Beginning rate of pay if paid work based learning experience • Statement of assurance from the employer of non-discrimination • Statement of assurance from the school district of compliance with non-discrimination www.rpmoll.com
What’s required in an approved training plan? • For unpaid learners, new specific skills listed for each 45 hours of the placement • Identify related academic courses (non-CTE & non-SE only) pupil currently or previously enrolled in • Signatures of: student, parent/guardian, employer, coordinator, & principal or designee • The following items are required, but will not be reviewed by the auditor • Teacher verification that the pupil’s placement relates to educational development plan. For CTE students, verification must be made by the vocationally certified instructor • List of performance elements that contribute to pupil’s progress & assessment www.rpmoll.com
