  2. How Healthy is Our Community? Developed MAPP Priority Indicators • Long-term indicators to be measured over time (years) • Can’t necessarily be addressed by one agency or sector alone • ~5 per priority Indicator considerations: • Sample size and population • Timeliness–is the data recent? Is collection of data ongoing and are there planned data releases? • Geography–is the data at a local level (county or smaller) and is it available for all district localities? • How well does it address the priority? • Overall balance between indicators within the priority

Supplement with New Data TJHD Community Health Survey • Mailed to 3,000 households • 934 respondents • Questions related to MAPP priorities (healthy eating, active living, mental health, substance use, aging, housing, etc.) Results will be available within the next few weeks and can be reviewed at the February meeting

Promote Healthy Eating and Active Living • Adult Obesity Rates • County-level estimates of BRFSS data from CDC; published by County Health Rankings • Physical inactivity • "During the past 30 days, did you participate in any physical activities other than your regular job?“ (County Health Rankings) • THRIV Obesity Rates • BMI category rates by zip code from UVA medical records for adults and children for the years 2014, 2015, and 2016

  8. Promote Healthy Eating and Active Living 4. Access to Exercise Opportunities • "Percentage of individuals in a county who live reasonably close to a location for physical activity" (County Health Rankings) 5. Adult and Child Food Insecurity • Households without consistent access to adequate food (Feeding America estimates using a model based on American Community Survey data)

Serious Mental Illness Hospitalizations • Hospital discharge data is supplied by Virginia Health Information (VHI) to Community Health Solutions which analyzes it for Region Ten in the Behavioral Health Atlas 7. Substance Use Disorder Hospitalizations • Same data source as SMI hospitalizations 8. Suicide Rates • Office of the Chief Medical Examiner (OCME) releases an annual report each year that includes counts and rates

  10. Address Mental Health and Substance Use 9. Poor Mental Health Days • “Thinking about your mental health, which includes stress, depression, and problems with emotions, for how many days during the past 30 days was your mental health not good?” 10. Smoking Rates • Changes in the methodology used by the CDC to create this measure in 2016 and 2017 means that new estimates should not be compared with earlier years 

  11. Improve Health Disparities and Access to Care • 11. ALICE Report –Asset Limited, Income Constrained, and Employed • A measure for the cost to live in a community at a basic "survival" level and a "stability” level (United Way) • 12. Poverty Status of Families • Percentage of families earning less than the Federal Poverty Limit (Census) • 13. Health Insurance Coverage • Insured and uninsured by age, sex, or race/ethnicity (Census data)

  12. Improve Health Disparities and Access to Care 14. Low Birth Weight and Very Low Birth Weight Births • VDH Division of Health Statistics has data by county and race (B/W) • TJHD has complete dataset of death records and is interested in a more in-depth project 15. Life Expectancy • TJHD has completed 2008-2012 life expectancy at birth estimates by census tract using the SCALE methodology

  13. Foster a Healthy and Connected Community 16. Free and Reduced Lunch • Virginia Department of Education (VDOE) publishes rates each school year during the fall 17. On-Time Graduation Rates • Students who entered the ninth grade for the first time together and successfully graduated four years later (VDOE) 18. 3rd Grade Reading SOL Pass Rates • Available by county, school, and a variety of demographic categories (gender, race, English learner, homeless, disabled, disadvantaged)

  14. Foster a Healthy and Connected Community 19. Vehicles Available • By household. Transportation indicator on who bikes, walks, v.takes public transportation to work (Census) 20. DSS Benefits • SNAP, TANF, Medicaid/FAMIS benefits by county by age, race, ethnicity, and gender (Virginia Department of Social Services)
