Oxidative Stress • Reactive oxygen species (ROS) • Oxygen ions • Free radicals • Peroxides • Antioxidant defense • Damage sperm membrane and motility • Alter sperm DNA and initiate apoptosis
Male infertility • Half of all cases of infertility • Affects one man in 20 • Reduction in seminal plasma and spermatozoa glutathione peroxidase • Reduction in non-enzymatic antioxidant activity in seminal plasma • IUI or ICSI, not for oxidative stress
Leukocyte • Positive correlation between seminal leukocytes number and ROS production • No significance difference between fertile and infertile men • Positive correlation between seminal ROS and cytokines (IL-6, IL-8, TNF-α)
Sperm • ROS assists the sperm’s transit through the cumulus and zona pellicida • ROS production ability inversely correlates with maturation state • Excess cytoplasmic residues in the mid- piece are rich in G6PD NADPH production • Produce less ROS (1/1000) but cause more DNA damage
Identification of oxidative stress • The cost and complexity of testing • The lack of a single standardized measure of oxidative stress • Direct method: damage created by ROS • Indirect method: ROS production • Routine semen analysis: asthenozoospermia, hyperviscosity of seminal plasma, seminal leukocytes, poor sperm membrane integrity
