  2. Riggers w/o Life Line Riggers with Life Line Observation- Riggers were working w/o Life Line on Mezzanine. ArshiyaInternational,Panvel Severity 5 Corrective Action- Stop work and provide life line.

  3. Observation- Gas Cutting set w/o Flash Back Arrestor/Fire Extinguisher. Arshiya International Severity 5 Corrective Action-Flash Back arrestor & fire extinguisher provided.

  4. Observation- Domestic Electrical Board w/o ELCB use.Arshiya International Severity 5 Corrective Action-Stop work till Electrical Board Replace with Industrial Plug Sockets with ELCB.

  5. Observation- Sheeters ascending descending help of bamboo structure & non std. Full body harness & some w/o full body harness. Arshiya International Severity 5 • Corrective Action- Stop work till access to roof provided.

  6. Severity 5 • Observation- Domestic Electrical Board with non-std. stand use on roof. Welspun Corrective Action- Electrical Board & stand Removed.

  7. Observation- Loose sheet bundle on roof at Welspun. Severity 5 Corrective Action- Sheet bundle fasten

  8. Severity 5 • Observation-Riggers sitting on cross bracing for fixing it. Jindal Rail Corrective Action-Stop work & instruct builder arrange boom lifter.

  9. Severity 5 • Observation- Sheeters Ascending & Descending w/o anchorage. Jindal Rail Baroda. Corrective Action-Fall arrestor provided & training given to all workers.
