Employee had work restrictions from a previous injury, which the employee did not follow. • Lessons Learned/Corrective Actions: • Need to practice safe-lifting techniques. • Medically prescribed restrictions must be followed at all times(even when they are not related to a work injury) until the prescribing physician releases to full duty.

Employee attempted to break fall by holding on to picking cart. Sustained multiple sprains and contusions from fall and by trying to brace self on cart. • Lessons Learned/Corrective Actions: • Lack of housekeeping and disregard toward debris on floor.
Employee Notice of Incident • Incident Location: • New Zealand • Type of Incident/ Injury: • Recordable – Light Duty (2 days) • Department/Job Function: • Forklift operator • Incident Description: • A forklift driver was parking his machine in the forklift park "P2" in the charging room. As he turned into the parking space, the machine drove over some water on the floor, which caused the machine to slide towards the wall. As the machine slid, the drivers foot was displaced and swung out of the machine. The machine kept sliding and collided with the wall, squashing the drivers foot between the machine and the wall. Driver can still walk and has full mobility of foot. The only thing that saved the driver's foot from more serious injury was his steel capped boots. Diagnosis: bruising and swelling of the foot. • Root cause: • Slippery/Wet floor, low lighting, fuse for lighting tripped off, driving speed, incorrect driving position (suspect right foot could have been raised and hanging out of the machine in a resting position). • Lessons Learned/Corrective Actions: • Wet locations must be cleaned immediately.Electrician to check fuse for lighting.Drivers to be trained in correct driving position.

By falling, his knee struck the driver door.In the morning, the knee swelled and became very painful. • Diagnosis: sprain. • Root cause: • Presence of black ice after a night of icy rain. • Insufficient salting by the Security officer. • Lessons Learned/Corrective Actions: • Reminder to apply salt in case of ice.
