What is the Physician Strategy? • One year planning grant from California Endowment: $142,000 • Program development effort focused onimproving the health and well-being of elderly and disabled persons by expanding the community's ability to • promote healthy behaviors, • manage chronic conditions, and • create new relationships in health care where consumers and professionals work better together.
Strategies & Objectives • Build on MFFS thinking that has emerged in other states • Partner w/physicians vested in chronic care • Develop interest/incentive for support of “after office” services (HCBC) • Identify care management/MD incentive resources • Train on healthy aging, geriatric/chronic disease protocol, pharmacy, HCBC supports
Strategies & Objectives…continued • 5-7 regional focus group meetings to • Engage PCPs in designing model • Engage CBOs in support of PCPs • Engage consumers/caregivers via health education
Planning Phase Deliverables • Develop Admin. Action Plan that would allow for successful implementation and evaluation of strategy in Phase II • Final Grant Report that includes comprehensive presentation of research, activity and methodology used
Physician Strategy Benefits • Improved Fee-for-Service model • Paradigm shift to chronic care mgmt • Responds to Olmstead Decision to provide greater consumer choice for LTC services • Potential to be expanded/replicated • Sustainability - Supported w/Medicare Advantage demo
Timeline • First meeting: today! Sign-up to participate • Identify additional physicians, consumers, caregivers, CBOs (2-04) • Official invitation to participate in learning sessions/focus groups (3-04) • Focus group meetings (4-04 thru 7-04)
Timeline…continued • On-going evaluation to synthesize feedback and develop implementation plan based on successful outcomes • Regular updates to Planning Committee • Complete Phase II implementation plan and Final Grant Report (12-04)
