Case Management & Anticoagulation Patients • Provision of annual in-services for case management (CM) to familiarize staff with: • Anticoagulants • Collaboration with anticoagulation management service (AMS) • Assistance programs available for patients AMS identifies all patients that will potentially be discharged on problematic/expensive anticoagulants via daily report AMS documents on anticoagulation daily monitoring form and also verbally notifies primary team that CM consult is required for patient prior to discharge • CM: • triages patient’s insurance • enrolls patient in assistance program if needed • completes paperwork and provides information/voucher to patient • documents actions in electronic chart
