Clinical training envelope • For each job accessed in this ARCP • green forms (training post assessment ) • yellow forms (trainee assessment ) • Two clinical supervisors then two each of above forms • NB At the end of each post: • 1. 14. Keep in your portfolio • Your Monday meeting attendance. • Every Monday must be accounted for, including annual and study leave. • At worst 80% attended, 20% valid excuses • Email Susan White in advance of the leave • Bring a copy of that correspondence with you • ST2 attendance is only compulsory if your job plan allows it • Other information that you want the panel to consider.
