Exit Process Full Exit Paperwork includes: Final Timesheet, Exit Form, Final Appraisal, Member Exit Survey along with Interest Accrual & Child Care Termination Forms (if applicable) • Direct Deposit ends with the 8/31/04 payroll for all members except those with commitment letter on file with National for a second term • Program Officer enters final timesheet prior to the member’s end date • This does mean that some members may have to project hours and obtain supervisory signature prior to the end of the pay period • E-mail Stacey when final timesheet is entered for approval • Stacey will e-mail you when final time sheet is approved for you to prepare exit form in WBRS and save as “Complete – Awaiting Approval” • Email Stacey when the exit form is completed • Exit process should be completed 4 days prior to pay day so that Cecilia can mail checks • Final checks will not be mailed unless exit process has been completed

Program Start-Up • Provide Site Information (8/10) • Site name, address, phone number, fax number and actual work site location if different from site address • Executive Director & Supervisor names & e-mails • Job Description • E-mail Cecilia & Pearl copy of commitment letter for creation of member agreements & MOUs What Some Do -

Program Start-Up Enrollment • Use and send commitment letter to member • 25 day window from making commitment to approval of enrollment form in WBRS • Enter commitment into commitment tracker on WBRS • Enter member information profile and enrollment forms and save as “Complete – Awaiting Approval” • Email Pearl or Stacey for approval Payroll • Original copies (no faxes please) of all member eligibility and enrollment documents must be received for payroll to be activated – we suggest using the member file checklist to ensure all documents have been received • Curious as to what the deadlines are? Check out the calendar on the intranet for deadlines for original document submission

Program Start-Up • Payroll • Enrollment & Eligibility Documents include • Whatever form you would use to put a member on payroll • Completed AmeriCorps Application • Completed CNCS Enrollment Form • Drug Free Workplace Policy • I-9 with appropriate ID • Photo Release • Enrollment/Waiver – Health & Childcare • W-9 • Signed Member Agreement • Direct Deposit, if applicable • Forbearance, if applicable • Copy of GED certificate, if applicable • Completed GED Agreement & signed letter by Supervisor, if applicable • Completed Childcare Application & Documents, if applicable

  6. 2004 -2005 Calendar • August – September • Program Start-Up Info To NY by 8–10-04 • Member/Supervisor Local Orientations • Program Description – Due 9/30 • September – October • Member Enrollment/Exit, Member Agreement Processing • National Orientation - week of 10/17 • 1st Desk Audit conducted • Draft Member Objectives for 2004 -2005 due 9/30 • Final 2003 – 2004 Progress Report due in WBRS 10/15 • November • Final Member Objectives for 2004 – 2005 Service due 11/15 • December • Performance Measures available for field 12/15 (contingent on all final member objectives being received 11/15)

  7. 2004- 2005 Calendar • January • APR Maps approved by NY by 1/15 • 2nd Desk Audit • RFP to field for 2005-2006 • February - April • Expression of Interest Due to NY • 1st Progress Report due 1/30* (Subject to change based on CNCS) • May - June • 3rd and Final Desk Audit • Site Visits • Notice of Awards & Slots • July • Site Visits • 2nd Progress Report due 7/31
