Solution - partnership • Integrated care / urgent care partnership • NHS Salford • Practice Based Commissioning Consortium • Salford Community Health • Salford GPs (as service providers) • Salford Royal Foundation Trust Other partners - NW Ambulance Service, Salford City Council, Greater Manchester West Foundation Trust

  7. Solution – service delivery • Joint project board • Service redesign • New payment model with clear incentives / disincentives • Discussion with the public • Relevant financial arrangements

  8. Current projects • Urgent Care Centre, Trauma and Resus (A&E triage, A&E assessment and rationalise access) • Optimising services and pathways (Frequent flyers, frail patients, end of life care) • GP access Chronic disease management “Patient education”
