CHWs deployed • 15 CHWs – Support / ongoing training • Mapping / registration of Ward 11 • Progress (15 CHWs) • Lots: 2703/Families identified: 6457/Fam/Lot: 2.4 • Fam reg:2340 /Pplreg: 10046 /Ppl/fam: 4.5 • Screening: • Challenges: social structure-dynamics, language-history, infrastructure, economics, politics, law, safety

  4. Stakeholders engaged • Monthly Meeting • Priorities: communication, confidentiality, food security, poor family support, environment, alcohol-drugs, unemployment, overcrowding, transport, elder care, migration

  5. PHC Re-engineered • Simple clinic – basics! • Full range of services • Family / Community-based records • Integrated Chronic Disease Mgmt+ • CHW core to process and teamwork • Challenges: Management (HR-Fin), ICT, Infrastructure, Referral, Teamwork, Systems

  6. Health Promotion • Coordinator: NkosinathiMakhubo • Plans • Awareness Campaign (incl. Services) + Screening • Daily CHW Walking Club • Weekly CHW Health Literacy Group • School Health • Challenges: Behaviour change, peers, social structure, politics,empowerment

  7. Future? • Students for 2015: • 4-5 GEMP4 per 6 weeks • 18 GEMP3 x 2 weeks (Public Health Block)? • 300 GEMP1/2 x 6 visits per year (10 per CHW pm)? • 8 Speech Therapy x 1 day/week – 6-7 weeks x 4 • OT students • Sustainability • FP / CA employed by GDOH • Request to mgmt to take over 20 CHWs + 0.5 PHCN / 1.5 EN by March 2015 – under consideration • Stephen/Shehnaz – HURAPRIM ends Feb 2015 • NIH grant failed  exploring re-send + others
