PHC Re-engineered • Simple clinic – basics! • Full range of services • Family / Community-based records • Integrated Chronic Disease Mgmt+ • CHW core to process and teamwork • Challenges: Management (HR-Fin), ICT, Infrastructure, Referral, Teamwork, Systems
Health Promotion • Coordinator: NkosinathiMakhubo • Plans • Awareness Campaign (incl. Services) + Screening • Daily CHW Walking Club • Weekly CHW Health Literacy Group • School Health • Challenges: Behaviour change, peers, social structure, politics,empowerment
Future? • Students for 2015: • 4-5 GEMP4 per 6 weeks • 18 GEMP3 x 2 weeks (Public Health Block)? • 300 GEMP1/2 x 6 visits per year (10 per CHW pm)? • 8 Speech Therapy x 1 day/week – 6-7 weeks x 4 • OT students • Sustainability • FP / CA employed by GDOH • Request to mgmt to take over 20 CHWs + 0.5 PHCN / 1.5 EN by March 2015 – under consideration • Stephen/Shehnaz – HURAPRIM ends Feb 2015 • NIH grant failed exploring re-send + others
