At the conclusion of this 12-minute presentation, you will understand the basics on the following topics: • Your benefits options; • Whom to contact with questions; • How to complete the online enrollment.

  5. Topic 1 What are my choices regarding employee insurance benefits?

  6. Automatic Benefits • From TRS (Teacher Retirement System of Texas) • Death benefit – about 2x salary • Disability benefit – only if not likely to ever return to work • Much more info at TRS website: http://www.trs.state.tx.us • $25,000 basic life insurance from FISD

  8. Dental Insurance Carrier: Ameritas

  9. Voluntary Benefits: Dental • Purpose: to help manage costs of routine and un-expected dental care • Type 1=100% U&C* • Type 2=80% U&C • Type 3=50% U&C • *Usual & Customary: average cost of a procedure for a geographical region

  10. Voluntary Benefits: Dental • Rates per month: • Emp only =$24.28 • Emp + 1 =$49.92 • Emp + fam =$74.92 • Dental Rewards • Roll forward unused benefit, $250/yr • Extra benefit for problem years

  11. Vision Insurance Carrier: Block Vision of Texas

Voluntary Benefits: Vision • Purpose: to help manage cost of routine eye care and vision correction • Network Providers offer greater savings – 18 providers w/in 50 miles of 75840 (Mexia, Groesbeck, Palestine, etc) • Out of Network Providers – still get a benefit • Provider list accessible through blue benefits website

  13. Voluntary Benefits: Vision • Rates per month: • Emp only =$5.85 • Emp + 1 = $9.80 • Emp + fam =$14.40 • In Network you get: • Standard lenses covered • Most frames covered • Contact benefit $150 • Copay $25/$25

  14. Long Term Disability Insurance Carrier: UNUM

  15. Voluntary Benefits: Disability • Purpose: to make sure you still have a check coming in when dr says you cannot work from disability • Accident • Sickness (including pregnancy)

  16. Voluntary Benefits: Disability • Elimination Period • How long you agree to wait before benefits kick in • 0/7, 14/14, 30/30, 60/60 • First Day Hospital with Elimination Periods of 30 days or less

  17. Voluntary Benefits: Disability • Benefits come to you usually once a month, like a paycheck • Use the money to pay for groceries, car payment, mortgage, medical bills – whatever you need • Rates not age banded. • Coverage does not automatically increase with salary increase.

  18. Cancer Insurance Carrier: Central United

  19. Voluntary Benefits: Cancer • Purpose: to provide money to help pay the high expenses associated with cancer treatment. • Medical insurance helps, but cancer insurance pays in addition to medical.

  20. Voluntary Benefits: Cancer • Rates per month for new policy (thru age 64), Plan B: • Emp only =$30.55 • Emp + ch =$33.96 • Emp + fam =$49.11 • Rates increase at 65 • Cannot buy new cancer policy age 70+ but can keep old policy

  21. Group Term Life Insurance Carrier: American United Life Insurance Company

  22. Voluntary Benefits: Group Term Life • Purpose: to help you or your survivors pay for final expenses plus unexpected loss of income and more • Low cost • Max 5x salary up to $500,000 • Spouse Benefit up to 50% of ee benefit

  23. Voluntary Benefits: Group Term Life • Eligibility Period = first 31 days after hire • Guaranteed increases to qualified employees • Employee and spouse rates age banded – every five years • Child coverage $10K each child for $2.60 per month regardless how many kids

  24. Medical Insurance Carrier: TRS ActiveCare

  25. Voluntary Benefits: Medical • Purpose: to help you manage cost of doctor and hospital visits, medical tests, medical emergencies, treatment of diseases, etc. • This enrollment is done on the HUB. • Questions go to Lisa Akers.

  28. Topic #3 How do I complete my online enrollment on the computer?

  29. Online Enrollment • Enrolling is easy! • Check schedule for your campus date • Log on to the internet • Go to your district’s benefits website – located at bottom of handout • Scroll down to lower left corner to find link to The Benefits Hub

Online Enrollment • To see instructions, click “Please Read First” • To begin enrollment, click “Begin Your Online Enrollment Here” • USERNAME: first 6 letters of last name, first initial of first name, last 4 digits of SSN • PASSWORD: your SSN • If you need to log out, the system will remember what you did already.

  31. Online Enrollment • Need help during the enrollment? • An Achieve agent, Lisa White, will be on each campus on the scheduled dates • You can enroll on your own or come see Lisa • You can also email Achieve with questions prior to the enrollment.

  34. Other Important “To Do” Items • Bring your family’s SSN’s this year to enroll – new requirement • Paper Forms MUST be turned in for: • Cancer • Voluntary Life • Bring signed forms to Lisa White on scheduled date
