History • 5 week history of RF lameness • Observed running around pasture the night of July 4th due to fireworks • Lame on July 5th • Initially swollen to level of the carpus • Treated at home with cold hosing, foot soaking, phenylbutazone, and rest. • rDVM blocked lameness to an abaxial nerve block • No abnormalities noted on rDVM radiographs • Improved markedly in the week before referral.
Physical Exam • T – 100.6 P – 36 R – 20 • Moderate effusion of DFTS • Sensitive to palpation • Mild effusion RF fetlock and coffin joint • Grade 3/5 lame on RF • Strong positive (3+) to RF distal limb flexion • 90% improvement with RF DFTS block
Ultrasound of RF DFTS • Moderate effusion of the DFTS
Treatment • Based on physical exam and MRI findings navicularbursoscopy with debridement of the DDFT lesion was elected. • Pre-op medications • K-pen – 22,000 mg/kg IV • Gentamicin – 6.6 mg/kg IV • Phenylbutazone – 4.4 mg/kg IV
Stem Cells • Bone marrow was harvested for stem cell collection • Culture takes approximately 3 weeks • Stem cells injected into DFTS by rDVMonce received
Post-Op Care • Hospital • K-pen – 22,000 IU/kg IV QID for 5 days • Gentamicin – 6.6 mg/kg IV SID for 5 days • Phenylbuazone – 2.2 mg/kg IV BID for 5 days • Bandage change every 2-3 days • Discharge • TMS – 30 mg/kg PO BID for 7 days • Phenylbutazone – 2.2 mg/kg PO BID for 3 days; then 1.1 mg/kg PO BID for 3 days • Bandage change every 3 days until sutures removed 14 days post-op
