Coordinated, evidence-based, high-quality care is better for those we serve. The Clinical Effectiveness Quality Pillar goal is to create the infrastructure to deliver care as a cohesive health system rather than a series of individual encounters. Why are we doing this?
BridgeBasics Bridge is Vanderbilt University Medical Center’s campaign for population health/payment reform initiatives. All population health/payment reform-related initiatives will be part of Bridge and will be differentiated by Phases. What is BRIDGE? Bridge’s core goal: better patient care. Each quality metric in Bridge is designed to prevent major health problems such as: • Diabetes complications • Colon cancer • Breast cancer • Adverse medication interactions • Pneumonia • Influenza End goal: Improved patient care
Bridge Basics What will bridge cover? • Bridge will be used to unify all current and future population health/payment reform-related initiatives, including: • Pay for performance (P4P) outcomes and accountability • Design and execution of population health analytics • Design and implementation of population health informatics capabilities • Episodes of care (Bundles) oversight • Care coordination deployment and assessment • Rationalizing patient site of service (patient flow) across the network • Design and deployment of transformative chronic care methods and potential other care methods targeted to segments of the population
StarTrackerIndicator Panel(in StarPanel) The StarTracker Indicator Panel is a platform for clinicians to record contraindications, outside screenings, and patient refusals related to quality metrics.
Outpatient Visit Panel (in StarPanel) The Outpatient Visit Panel shows services needed by patients related to key quality metrics as well as upcoming (or current) appointments.
Bridge Quality Dashboard(in Tableau) The Bridge Quality Dashboard shows up-to-date snapshots of quality performance and targets; it includes Clinic and Clinician detail views.
