Wisconsin Children in Out-of-Home Care Annual Report for Calendar Year 2012 December 31, 2013 • 259 children entered out of home care – 3rd highest in the state behind Milw and Madison • 6.15/1000 children – 10th highest in the state • 34% under age 5 years • 30% age 11-16 years
3/13-2/14 Kenosha Co • >25 months to TPR • Almost 7 years in OHC until aging out • have >2 placement • 16% in the 1st year • 36% by 2 years • 62% if in the system >2 years
How do Foster Care and Care4Kids relate to KIC? • The vaccine years • Unstable homes mean inconsistent healthcare and lost records
Care4Kids • A Medical Home for Children in Foster Care • Centers of Excellence • Communication and integration of resources • Comprehensive coordinated collaborative care
What is the goal of Care4Kids? • This new approach to healthcare will: • Strengthen quality of care • Increase access to care • Improve timeliness of care • Improve physical and mental health outcomes for children • Promote improved resiliency • Create stronger natural support systems for children in Out-of-Home Care
What benefits will children enrolled in Care4Kids receive? • An Out-of-Home Care Health Screen within 2 business days of entering care • A Comprehensive Initial Health Assessment within 30 days of entering care • A mental health evaluation (if needed) • All Medicaid-covered benefits
Dental and Vision care • Ongoing routine check-ups • A Health Care Coordinator • Preferred access to Child Advocacy Centers and Centers of Excellence • A Comprehensive Health Care Plan tailored to the child’s individual health needs • Ongoing Health Care for an additional 12 months after discharge from out-of-home care
