Purdue Cup 2009 Entry Form October 24th, 2009 Name__________________________ Address_________________________ ________________________________ Affiliation(School)_________________ Doubles Partner___________________ Skill Level_____ Price (Doubles Included) $15 Purdue Racquetball Club member $35 Everyone Else Format: Olympic Format for Singles and Doubles (Doubles will only have Gold and Red). Singles will be started in the morning following by doubles. Tournament Director: Kelly Gremley You can sign up via email (preferred method) at kgremley@purdue.edu , calling (847) 899-1337, or sending this form to: 415 Trace 4 West Lafayette, IN 47906 Entries must be received by October 21st. • Open to all players!! • You are guaranteed 3 singles matches. • Lunch provided • Awards for singles • 1stLarge mug • 2ndMedium mug • 3rd Shot glass • 1st and 2nd only for Doubles • Possible Collegiate Team event (based on top 6 players from your school) minimum of 3 players. • You will be required to sign a waiver before playing. • Must have eye protection. • May only enter on doubles event. • This is not a USAR/INSRA sanctioned event. • All skill levels welcome!

Do not try to park in other places closer to campus since you may be towed or ticketed; there is a HOME FOOTBALL GAMEthat takes up most of the street/lot space!
