North Country Children’s Clinic Founded by Volunteers & First Well Child Clinic Held Today, NCFHC operates 3 community locations with integrated services; 6 SBHCs with an additional 10 school-based dental sites • Prenatal Education Program Founded; Now Known as Healthy Outcomes • Pediatric Dental Care Services Commence School-Based Dental Program Commences Transitioned to FQHC, now serving all ages as North Country Family Health Center; Went Live with EHR 1971 1974 1980 1989 1991 1993 1994 2010 WIC Program Funding Sought and Received; One of First in the Country 2012 • Pediatric Primary Care Services Commence 2019 • Pediatric & Family Behavioral Health Services Commence; Start of School-Based Health Program • TelemedicineProgram Begins

What is a Federally Qualified Health Center? • A community-based healthcare provider that receive funds from the Health Resources & Services Administration to provide primary care services in underserved areas. FQHCs must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes a majority of patient members.

Community-Based Primary Care • Located at 238 Arsenal Street, Watertown • Family practice • Pediatrics and Adults • Internal Medicine • Prevention, diagnosis, and treatment of adults • Telehealth (primary care, teleretionpathy) • Medication Assisted Treatment (MAT) services • Provided as a treatment option under our Primary Care providers for our patients • Behavioral Health/Substance Use Care Coordinator • Integrated services • Four adult providers (2 MDs, 2 NPs) and two pediatric providers (1 MD, 1 NP)

State Street, Lowville • 2 dentists; 2 RDHs • 26908 Independence Way, Evans Mills • 3 dentists; 3 RDHs • Preventative and Restorative Care • Teledentistry – Pediatric patients only for oral surgery consultants and follow-up

State Street, Lowville • 26908 Independence Way, Evans Mills • Nutrition, breastfeeding support, monthly checks and community referrals • Facebook page for information on satellite locations (Jefferson County WIC and Lewis County WIC)

  16. Healthy Outcomes • Services for high risk pregnant women and their families through 1 year postpartum • Case management • Education • Baby supplies • Community referrals

  17. Facilitated Enrollment • Certified Application Counselor • Assists patient with obtaining Medicaid and health insurance on the New York State Exchange and with NCFHC’s sliding fee scale application • Assists approximately 4,000 individuals with insurance enrollment annually
