What is a Federally Qualified Health Center? • A community-based healthcare provider that receive funds from the Health Resources & Services Administration to provide primary care services in underserved areas. FQHCs must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes a majority of patient members.

Community-Based Primary Care • Located at 238 Arsenal Street, Watertown • Family practice • Pediatrics and Adults • Internal Medicine • Prevention, diagnosis, and treatment of adults • Telehealth (primary care, teleretionpathy) • Medication Assisted Treatment (MAT) services • Provided as a treatment option under our Primary Care providers for our patients • Behavioral Health/Substance Use Care Coordinator • Integrated services • Four adult providers (2 MDs, 2 NPs) and two pediatric providers (1 MD, 1 NP)

Facilitated Enrollment • Certified Application Counselor • Assists patient with obtaining Medicaid and health insurance on the New York State Exchange and with NCFHC’s sliding fee scale application • Assists approximately 4,000 individuals with insurance enrollment annually
