  2. Objectives • Discuss a methodology to assess community health needs based on community health disparities • Discuss methods to engage community members, i.e. focus groups, etc. so as to identify community health services wanted by the community • Discuss program planning and implementation to meet the community health needs and the wants of the community. • Discuss a program that was implemented to meet a community need and has shown evidence of success in reducing a disparity

Community Assessment • Identify the sequence and what needs to be included in a community assessment • Instill the ideas of how you can include the community in doing their community assessment • Use of data to improve and to provide great care – remember – it takes a lot of time to plan a party • Develop an action plan (remember soon is not a time!)

Community Assessment • What, by when, and how • What are we trying to accomplish – a good solid community assessment so we can identify what the community wants and needs and how we can make care better • How will we know care is better • What steps need to be completed – what pieces do I have to do to get it done • Outline

Community Assessment • Who can help us get it done – who can help with this piece – who can we reach out to • Develop a Gant Chart to keep on track • Where do we want to be by this date i.e. LS3 – 3 months (June) • Do small tests of change, i.e if sending invitations to community members – draft, run the draft by 1 or so persons or a community member, make changes, then send
