Goals • Debrief, review results and discuss plan, team will break up into three groups. • MiSiS group, phone group, and enrollment verification group. • Contact families to investigate the reason for students being on the potential dropout list, students being chronically absent, or students being on the no show list. • Contact schools based on information provided by parents/students to verify enrollment. • Gather information about: • The student’s current enrollment status if “No Show” or “Dropout” • Reasons for history of poor attendance if “Excessive Absence” • Recommend a follow-up conference with the Pupil Services staff member or designated staff at the school to discuss options and resources as appropriate • Make an attempt to have family commit to a scheduled parent conference
Post AMD Team Packet • All intake forms from home visits and phone bank • Phone Call Tips and Reminders • Student Information Recommended: • MiSiS-Attendance Referral Report for each identified student • Intake Form • Enrollment Verification Form • Appointment Calendar • Resource Matrix • Education Options Flyer
Post AMD Team Procedures DO: • Introduce yourself and explain the purpose of your call • Be mindful and respectful of families at all times • Use suggested script to initiate conversation with family • Document all family responses on Intake Form • Verify the parent/guardian’s name • Document any changes in the family whereabouts • If the parent/guardian is not available leave a general message to call back. Leave school contact information DO NOT: • Become confrontational with the parent/guardian or student • Discuss student information with other family members. If anyone other than parent/guardian is available, leave the school contact information
Guidelines for follow up calls Call all of the numbers listed on the student’s attendance record (Attendance Referral Report) until you are able to establish contact with a parent or guardian • Introduce yourself with your name, and state that you are calling on behalf of the school • If no one is available, leave a message requesting a call back to the Pupil Services staff member or designated staff at the school • Make note of any numbers that may no longer be working on the intake form. • Provided necessary forms with documentation to MiSiS group to update leaver codes and document notes in MiSiS
Potential Dropouts • If the student is not enrolled: • Ask the parent how long the student has been out of school, and why? • Offer available resources if appropriate • Counseling • Parenting classes • Substance abuse rehabilitation • The Pupil Services staff member is available to help. Please encourage the parent or guardian to call or come in for a conference.
If Student Moved Out of Area • Ask for: • The name and relationship of the person providing the information • New contact information for the family, including the city and state of residence • New school information • Name of school • City, State • Phone number • Approximate date of enrollment • Fax enrollment verification form to new school
Enrollment Verification Team • Review all enrollment verification forms are complete. Look for SSID, non-lausd school must have matching SSID • Get school contact info, contact person and method of delivery: email, fax or mail. • Verify receipt of Enrollment Verification • Provide info to MiSiS group to update leaver codes and counseling communication
Group updating MiSiS information • Verify information on intake form, Name and DOB must match. • In MiSiS under support tab, go to counseling communication and place notes • Update Leaver codes for students
Documentation • Please record all information on the Intake Form
Statistical Form • Pupil Services staff member or designated staff at the school complete the Statistical Form and email it to the person indicated on the bottom of the form.
