Partnership with Camden Coalition of Healthcare Providers • In 2011, UnitedHealthcare and the Coalition began a partnership by sharing information among providers in- and outside of Camden and aggressive, face-to-face care coordination with members.  • Led to: • lower costs • more appointments with PCPs • fewer visits to specialists • fewer prescriptions at a lower total cost • fewer hospitalizations.
