18 Month Well Baby Promotion Physician Rounds: • Brampton Civic, Trillium and Credit Valley hospital Family Practice Rounds • Presentation was offered to Chiefs of Paeds • Family Practice Physicians were provided a USB key with resources and links to Offord Centre site • Physician resource website is under construction
The Healthy Babies Healthy Children Program and community partners on the Early Identification Committee worked together to develop the Peel Pathway. Pathway will be revised, as needed, by the Early Identification Committee. The Early Identification Committee is Co Chaired by Peel Public Health and Infant Child Development Services of Peel (ICDSP)The Early Identification Committee meets quarterly and promotes agency collaboration and resource development in addition to the 18 Month Well Baby Pathway
