2. SUPPORT FROM TRIBAL APPLICANT Include all costs to be paid by the tribal 10% match. Describe what your tribe has in place to support the AIVR project.

What needs to be done and how long will it take the tribe to make facilities and resources fully accessible for individuals with disabilities?
5. Protection, Use, and Release of Personal Information [34 CFR 369.45 (b)] The use of information and records concerning individuals must be limited only to purposes directly connected with the project, including project evaluation activities. Information may not be disclosed, directly or indirectly, other than in the administration of the project unless the consent of the agency providing the information AND the individual to whom the information applies, or his or her representative, have been obtained in writing.

The final product of the project may not reveal any personal identifying information without written consent of the individual or his or her representative.

11. REASONABLE COSTS TO SERVE CONSUMERS Let the federal and non-federal budget reflect exactly what you propose in the narrative and be able to justify all requests for funding and tribal support. Are the costs adequate to serve all eligible consumers and result in gainful employment?
12. Consumer Results and Benefits Number of consumers to be served annually under an IPE Number of consumers to achieve an employment outcome annually Number of consumers who retain or maintain employment Do the consumer numbers change from one year to the next; how do you justify this change?
