Required Documents • Admission Form • 2 x 2 Photograph • Original birth certificate and copy (issued July 2010) • Original Social Security Card and copy (Law 186, September, 1st, 2006)Copy available for review in Admissions Office • Pediatric Health Certificate • Immunization Certificate (PVAC-original form) (Law 25, September, 25, 1983)Copy available for review in Admissions Office

Required Documents • Vision Evaluation(by an Optometrist/Ophthalmologist) • Hearing Evaluation (evaluation of a hearing screening by a medical doctor in Audiology) • Behavior Certificate (by a teacher or director of the current school) • Official school transcript from the previous school with a minimum 2.50 average in a sealed school envelope • Official school transcript of the current year with the promoted grade in May with a minimum 2.50 average in a sealed school envelope • Kindergarten Diploma (for students entering First Grade) • Admission Test (Kinder – 11th) • Psychometric Evaluation or other when considered necessary.
