Regional Immunity in MelanomaImmunosuppressive changes precede metastasis Aaron Mansfield, MD Outrun the Sun Melanoma Research and Education Forum October 6, 2011
Hypothesis • Melanoma suppresses the immune system • Suppression promotes metastasis
T cells • T cells critical component of immune system • Cell-mediated immunity • Regulatory T cells • Th1: cytotoxicity • Th2: humoral response, inhibits Th1
Markers • Regulatory T cells Foxp3 • Th1 cells T-bet • Th2 cells GATA3
Lymph Nodes • Sentinel lymph nodes without melanoma • Sentinel lymph nodes with melanoma • Reactive lymph nodes • Dormant lymph nodes
Conclusions • Lymph nodes are suppressed prior to spread of melanoma • Both Th1 and Th2 transcription factors are downregulated • Controls can significantly impact interpretation
Questions • What drives immunosuppression? • Do these findings relate to survival or recurrent disease?
