Back to Main Page LTI Notification LTI No 01 2014 1th January Accident Location: Sea and Land in West-19, Burhan WHAT HAPPENED: The IP was holding onto a lifting sub for a 6 ½” drill collar to guide it whilst it was being lifted from its basket by the off driller side air winch. During the lift the IP lost his grip on the lifting sub, it slipped from his hand and swung towards the mast leg. The IP attempted to stop it striking the leg and in doing so his left hand was crushed between the lifting sub and the leg. The IP was taken to the rig clinic for initial treatment and then transferred first to Adam Hospital and then later to Nizwa Hospital where he was diagnosed as having fractured his left hand. • INJURY: • Hand fracture . • 6 weeks off work . Back to Main Page LTI Notification LTI No 02 2014 9th January Accident Location: Saxon Energy, Marmul WHAT HAPPENED: The IP was on the rig floor completing the nippling down of the blow-out preventer. After laying down the mousehole, the crew had placed a an ill-fitting temporary cover over it. The IP stepped on the temporary cover which moved causing him to slip, fall over and sustain an injury to his groin area. First aid was administered by the rig medic and the injured man was sent directly to Salalah hospital for further treatment on a deep laceration. • INJURY: • Groin. • 5 weeks off work . Х • √
Back to Main Page LTI Notification LTI No 03 2014 18th January Accident Location: Dalma Energy, Nimr WHAT HAPPENED: The floor man (IP) & driller were inside the cellar nippling up the Tubing Head Spool (THS) onto the Casing Head Housing assembly (CHH). Whilst attempting to align one of the bolts on the THS into the corresponding hole of the CHH assembly, the IP placed his left hand inside the gap between the THS and CHH assembly. When the bolt had aligned the THS dropped down onto the CHH and trapped IP’s left hand in between the two flanges. The IP was immediately sent to the PDO Nimr clinic. First aid was given and then he was transferred to the Salalah Hospital for further treatment. Back to Main Page LTI Notification LTI No 04 2014 24th January Accident Location: Salalah to Muscat road, Fatal motor vehicle incident A fatal motor vehicle incident occurred on the Salalah to Muscat road on Friday, 24 January, 2014 at approximately 15:45 hrs. The deceased was a 41-year-old Pakistani truck driver working for Ajib Trading LLC, a sub-contractor to our hoist contractor Shiv-vani. WHAT HAPPENED: The truck was transporting drill pipes from Shiv-vani’s hoist-21, operating in SaihNihayda, to the Sino Gulf base in Nizwa. En route the truck rolled over, which sadly resulted in the driver losing his life. An investigation into the incident is underway. Our thoughts and prayers are with the family, friends and colleagues of the deceased.
Back to Main Page LTI Notification LTI No 06 2014 04th March Accident Location: Al Noor MGI Phase 3A project ,Arabian Industries Projects,Nimr WHAT HAPPENED: The IP and an operator were working at height inside the bucket of a bucket truck. They were in the process of cutting temporary wooden supports from a pipe rack and had just moved the bucket into position beneath one of the wooden supports. While attempting to position themselves within the bucket to cut the support, the controls of the bucket were accidently engaged. The bucket moved trapping the rigger between the guard rail of the bucket and the pipe rack structure. The operator reacted quickly and managed to drop the basket both backwards and downwards to save further injury. The PTW Holder who was at location raised the alarm and an ambulance arrived to evacuate the IP to the construction camp clinic. He was later referred on to the PDO Marmul clinic and then on to the Salalah Hospital where he was diagnosed as having fractured his ribs. • INJURY: • IP having ribs fracture
Back to Main Page LTI Notification LTI No 05 2014 25th January Accident Location: Desert Byrne Drilling, Lekhwair WHAT HAPPENED: During a rig move the truck was transporting a diesel tank secured on a flat bed trailer from Lekhwair to Yibal. Whilst negotiating a sharp right hand bend he lost control causing the truck and trailer to roll over. Both the driver and his passenger were initially taken to the Yibal PDO clinic for their initial treatment and then transferred to Nizwa hospital for further examination and treatment where the driver was diagnosed with a fractured collar bone and dislocated left shoulder joint. The passenger suffered only minor bruises and was discharged with painkillers. Bend the lorry was negotiating • INJURY: • Fractured collar bone and dislocated left shoulder. • 3 weeks off work .
Back to Main Page LTI Notification LTI No 07 2014 22nd January Accident Location: Hoist 25 Nimr , Shaleem Petroleum WHAT HAPPENED: The IP was re-arranging a sewage pipe over the edge of the waste pit when he lost his balance and fell 1.5m down the slope into the pit and in doing so twisted his right ankle. At 18:30 pm he informed the HSE Advisor from the camp site who took him to the Sahara PAC Clinic where he received an injection and painkiller. On the 26th January the pain had not subsided and so we was referred to the Badr Al Sama Hospital in Salalah for a specialist X-ray which revealed a fracture in the right ankle. • INJURY: • Fractured right ankle. • 6 weeks off work . Back to Main Page LTI Notification LTI No 08 2014 22nd February Accident Location: Thulailat-151 well in the Marmul area, Vanguard WHAT HAPPENED: The incident occurred during the wireline logging operation. While the wireline operator was pulling the cable out of the hole, the IP climbed on the deck of the unit to clean the winch area. After cleaning one side, he attempted to cross to the other side between two wire drums. His foot became trapped and was crushed between the moving wire drum and a steel frame. The operator immediately stopped the winch and reversed it to free the IP’s foot. The IP was immediately taken to the Marmul PDO clinic for his initial treatment and then transferred to Salalah hospital where he was diagnosed as suffering from multiple fractures of his right foot. He was later referred onto Khoula hospital, Muscat for re-constructive surgery. IP’s foot was here • INJURY: • Multiple fractures of his right foot. • 8 weeks off work .
Back to Main Page LTI Notification LTI No 09 2014 26th February Accident Location: Ras Al Hamra project in Muscat, Al Turki WHAT HAPPENED: The IP was working on a balcony situated at roof height on one of the new director houses. Employees were protected from a fall from height by barrier protection which had been fitted at the edge of the balcony. The IP lent against the barrier which gave way and he and the barrier fell 10 metres down to the ground below. He was immediately given first aid and then transferred to Khoula hospital where he was admitted to intensive care. He sustained multiple fractures. • INJURY: • IP sustained multiple fractures admitted to intensive care IP fell from 2nd floor balcony IP landed on ground floor
Back to Main Page LTI Notification LTI No 10 2014 26th February Accident Location: Fahud, Midwesco workshop, Midwesco WHAT HAPPENED: A chief mechanic and a mechanic were removing a 18kg pin from a cross head and connecting rod for a JWS-400 pump using a press. As the chief mechanic (IP) operated the press which pushed the pin vertically down out of the hole the other mechanic crouched down and waited to catch the pin when it emerged fully from the hole. Unfortunately as the pin was greasy, as it came out he lost his grip on it and it fell 30cm down onto the left foot of the IP who was still operating the press. The crew administered first aid and then he was transferred to the Fahud PDO Clinic. He was then referred onto Ibri hospital where he was diagnosed as suffering from a fracture to his foot. • INJURY: • IP suffering from a fracture to his foot.
Back to Main Page LTI Notification LTI No 11 2014 25th February Accident Location: Al Mukhtar, a subcontractor of the 4PL logistics contractor for PDO WHAT HAPPENED: The driver arrived at Rig 7’s new location with a full load of pipes and casing joints. After he had unstrapped the load, the Truckoman rig moving crew began to unload it using a forklift truck. Whilst the unloading was taking place the driver at the back of the trailer attempted to remove wooden dunnage which separates different levels of the load and allows the forks of the forklift to slide under the piping. In the process of removing the dunnage he caused a pipe to roll which trapped his hand between the pipe and a trailer support stanchion. He managed to remove his hand but not before the crush had injured two fingers of his right hand. The driver was taken to the Marmul PAC clinic where the doctor treated him and then referred him to Salalah hospital for X Rays which diagnosed him as having a fracture of the middle finger. • INJURY: • IP having middle finger fracture
Back to Main Page LTI Notification LTI No 12 2014 13th March Accident Location: Cactus Premier Drilling, Fahud area. WHAT HAPPENED: Whilst securing tubular pipes on a trailer at MB completion workshop in Fahud, the IP lost his balance and fell from the trailer bed (1.8 m height). He was immediately transferred to PDO clinic for first aid, after which he was transferred to Nizwa Hospital for further treatment. X-rays showed forearm fracture and dislocation of the right elbow joint. The IP was discharged with a cast on the right elbow. • INJURY: • IP having fracture and dislocation of the right elbow joint.
Back to Main Page LTI Notification LTI No 13 2014 13th March Accident Location: Lekhwair, Oman Oil Marketing sub contractor of KCA Deutag. WHAT HAPPENED: The (IP) and his colleague (the driver, unhurt) were in their way back to Muscat after they have delivered engine oil drums to PDO Rig-11. The driver faced some sand dunes on the road where he was not able to see them from far due to windy and bad weather. The truck slipped out of the road when drove over the dunes and lost control. Police rescued them and took them to PDO Lekhwair clinic and later on both of them were sent by the ambulance to Ibri hospital. The driver was discharged after the medical examination but the helper suffered fracture of L1 vertebra and canal compromise due to fragments into the spinal canal. • INJURY: • IP having fracture of L1 vertebra and canal compromise Sand Dunes

WHAT HAPPENED: IP was carrying out analytical measurement when manhole cover blew out flashing the content of the chemical drain causing burns on IP eyes. IP is still admitted in hospital for further observation. • INJURY: • IP suffering chemical eye injuries.
Back to Main Page LTI Notification LTI No 15 2014 26th March Accident Location: SAIH RAWL, OFSAT . WHAT HAPPENED: An Ofsat oil field truck was approaching a left hand turn to Rig 85 which was based just off the Saih Rawl to Barik graded road. A diesel tanker had travelled along the road in the opposite direction and had left pockets of dust lingering in the air along the road. The junction to the rig was shrouded in dust and as the Ofsat truck turned left at the junction a Galfar 3 tonne canter approaching from the opposite direction collided with the truck which was across his lane. The canter driver and its two passengers were taken initially to the Galfar clinic and then to the Qarn Alam PAC Clinic before being transferred to Nizwa Hospital where X-rays revealed that the driver had suffered multiple fractures to both his legs. He had also suffered a strain to his left lower chest and his left elbow. One of the passengers also sustained abrasions on his neck and left knee. The second passenger of the canter and the Ofsat driver escaped without injuries. • INJURY: • IP suffering multiple fractures to both his legs and a strain to his left lower chest and his left elbow.
Back to Main Page LTI Notification LTI No 16 2014 31st March Accident Location: RIMA SMALL FIELDS, ABRAJ ENERGY. WHAT HAPPENED: After completing maintenance of the rig’s mud pump the crew turned it back on. The Derrick Man who was part of the mechanical crew noticed an abnormal noise coming from the pump and to identify the source of the noise he walked around the pump checking it. As he walked up to the pump’s external rotating crankshaft he was struck by a rotating metal bar which had inadvertently been left connected to the crankshaft. The bar had been used to manually rotate the crankshaft when it had no power and the crew had forgot to remove it after they had finished. He was knocked unconscious and was found on the ground by the rig crew. He was taken to the rig clinic for initial treatment and then transferred to Haima Hospital where he was diagnosed as having a fractured right leg and injuries to his left hand. The doctor referred him on to Nizwa Hospital for further treatment. • INJURY: • IP suffering fracture right leg and injuries to his left hand. Back to Main Page LTI Notification LTI No 17 2014 30th March Accident Location: Yibal, MB Petroleum Services . WHAT HAPPENED: After rigging down the Wireline equipment, the Wireline tools were being loaded onto a Hiab truck. During the loading process, the truck driver noticed that a 12” well head crossover had incorrectly been placed vertically on the back of the truck. He attempted to push the crossover over into a horizontal position with his left foot whilst his right foot was positioned on a lubricator. Despite holding the side of the truck for balance, as he pushed the cross over his right foot slipped off the lubricator, causing him to fall over and his left wrist struck another lubricator on the truck. He was immediately taken to PDO Yibal clinic for first aid treatment and then transferred to Ibri Hospital for further treatment where X-rays revealed he had fractured his left wrist. • INJURY: • IP suffering fracture of left wrist.
Back to Main Page LTI Notification LTI No 18 2014 02nd April Accident Location: Nimr, Dalma Energy Rig 47 . WHAT HAPPENED: During a rig up operation the crane operator needed to leave the crane operating cabin towards driving cabin. To do this he had to first step down onto the floor of the crane before climbing down the fixed ladders attached to the side of the crane to the ground. Whilst descending from the cab his left foot slipped and he did not have sufficient grip on the handholds to prevent his loss of balance. He fell 1.3 meters down to the ground, landing heavily on his left hand. He was taken immediately to the rig camp clinic for treatment and after consultation with the Marmul PDO clinic doctor he was transferred to Salalah hospital where X-rays revealed he had fractured both his left wrist and forearm. • INJURY: • IP suffering fracture of left wrist and forearm. WHAT HAPPENED: The Tool Pusher was on the rig floor in preparation for circulation and pulling out of hole. Whilst on the rig floor he needed to go and check the shakers but as he walked past the rotary table his footing slipped, his ankle twisted and he fell to the ground on the rig floor. He was immediately taken to the rig camp clinic for treatment by the medic and then transferred to Haima Hospital where X-rays revealed he had fractured his right lower leg. • INJURY: • IP suffering fracture of right lower leg.
Back to Main Page LTI Notification LTI No 20 2014 06th April Accident Location: Nimr NRPS, Wood Group CCC. WHAT HAPPENED: The IP and his helper were carrying out hot bolting on the fire water tank T4808 pipe line flanges at NRPS as pre-shutdown activity. The IP was initially hammering the impact spanner to loosen nuts while the helper was holding the combination spanner on the lower nut. The IP got tired and asked the helper to do the hammering (switching roles). Whilst the helper was hammering to loosen the bolts, the IP observed that the impact spanner was getting loose and instinctively attempted to put it back without communicating with the helper to stop hammering. The IP got his right index finger hit by the hammer and he was immediately taken to Tawoos clinic. After consultation with Marmul PDO clinic doctor, the IP was transferred to Haima hospital where X-rays revealed fracture of distal phalanx right index finger. • INJURY: • IP suffering fracture of distal phalanx right index finger. Back to Main Page LTI Notification LTI No 21 2014 26th April Accident Location: Rima, Weatherford. WHAT HAPPENED: The incident occurred when a joint of drill pipe with a scraper attached to it was lifted with the forklift, using a single webbing sling. The IP was guiding this load to be placed on supports on the ground when he stepped backwards and tripped over another joint of pipe. When he fell he tried to support himself on the sling which then came off the fork and the joint with the scraper fell on his left foot. He was immediately taken to the STS Amal clinic for first aid, and then transferred to Salalah Hospital where X-rays revealed he had fractured both bones in his left leg. • INJURY: • IP suffering fracture of left lower leg. Back to Main Page LTI Notification LTI No 22 2014 7th May Accident Location: Lekhwair, Al Bunyaan subcontractor for EMC. WHAT HAPPENED: The technician was repairing one of the A/C chiller units in the Lekhwair Pumping Station at substation 10 and in doing so he needed to check the belts tension. While doing that his left hand fingers got trapped between the belts and the pulley. This caused injury on three fingers of his left hand. He was immediately taken to Lekhwair PAC clinic for first aid, and then escorted to Ibri Hospital where diagnosed as Complete amputation of the distal phalanx of the left little finger. Partial amputation of the distal phalanx of the ring finger. He was taken by his company to Khoula hospital and then he discharged himself to continue treatment back in India. Back to Main Page LTI Notification LTI No 23 2014 7th June Accident Location: Yibal B station WHAT HAPPENED: During the regular maintenance the team believed that they had isolated and depressurised the launcher down to atmospheric pressure down from the 56 bar in the gas line. They then attempted to open the ring lock trap door at the end of the pig launcher but as they did so the door violently flung open. It swung right to left on its hinges striking the supervisor on the arms and then sheared off coming to rest 2 metres from the launcher. Two other members of the team received minor injuries as a result. The emergency response protocol was initiated, Yibal B was shut down and fire and ambulance services were mobilized to the site. The LECC was also activated. The three injured people were transported to the PDO clinic for treatment and stabilization before being transferred to Ibri Hospital for further checks and scans. The most severely injured person was further transferred to Khoula hospital where he was on operated on for a multiple fracture to his left arm and fracture to his right wrist. The other injured people sustained bruises and minor cuts to the face & chest. • INJURY: • IP suffering multiple fracture to left arm and fracture of right wrist. Back to Main Page LTI Notification LTI No 24 2014 25th May Accident Location: RAH school project, MAF. WHAT HAPPENED: At around 7:30 hrs the helper was walking across the new school building site and was carrying electrical tools in both hands. As he walked he lost his balance on a layer of loose sand which had covered a pavement. He fell down and as he was carrying tools he was unable to cushion his fall causing him to fall heavily on his left elbow. He was given first aid treatment immediately by the foreman and a HSE supervisor and transferred to the Apollo hospital in Muscat. In hospital he received surgery for his injury and remained an in-patient for 3 days. It is not yet established how long the helper will be absent from work as the contractor failed to notify PDO of the incident or injury. Appropriate action has been taken against the contracting company and all the individuals responsible or involved in the failure to notify the incident. An investigation is underway. • INJURY: • IP suffering left elbow fracture.
Back to Main Page LTI Notification LTI No 25 2014 9th June Accident Location: Carillion’s coastal yard, MAF. WHAT HAPPENED: Whilst the helper was helping offload steel pipes from the rear of a lorry in the Carillion’s coastal yard, his attention was distracted by a colleague. As he was not concentrating on his task he was unable to react when another colleague on the back of the lorry moved the pipes backwards trapping his thumb between the tail gate and a pipe. He was immediately taken to Khoula hospital where they diagnosed a serious injury which included a distal amputation and fracture of his right thumb. He was admitted to hospital and discharged 5 days later. He is expected to be off work for six weeks. • INJURY: • IP suffering right thumb distal amputation and fracture.

WHAT HAPPENED: The helper, working with a team of plumbers in the school administration block was cutting a groove into a wall with a grinder when the glove he was wearing snagged on some wiring. This led to him cutting the back of his hand with the grinder he was using. He was immediately given first aid and then transferred to the Apollo Hospital for minor surgery. He is expected to be off work for 42 days. • INJURY: • IP suffering a deep cut to his hand.

WHAT HAPPENED: The technician was performing a Preventative Maintenance work on Gas cooler fan motor, his index finger got trapped between motor pulley and belt causing deep lacerated wound in the right little finger. He was immediately taken to Qarn Alam PDO clinic where he received initial treatment and later referred to Nizwa hospital for further management. At Nizwa hospital, X-Ray revealed fracture of the tip of the finger, dressing was applied and injured person discharged on antibiotic and analgesic medications, he will be seen by Nizwa doctor after three days for further assessment. Back to Main Page LTI Notification LTI No 28 2014 8th June Accident Location:Saih Rawl, QAL. WHAT HAPPENED: Whilst descending the stairs from the tea room cabin he slipped and his grip on the handrails was insufficient to prevent him losing his balance and falling off the stairs an down to the ground landing heavily on his left hand and knee. He was immediately taken to the SMS-4 clinic for treatment by the medic and arrangements were made to transport him to the hospital at first light. At the hospital X-rays revealed he had fractured his wrist and bruised his knee. He is expected to be off work for two weeks. • INJURY: • IP suffering a fracture of left wrist and bruised knee.
