Health Center Patient Visit Flow Process with Routine HIV Screening, Rapid Test Dental Model Front Desk Front desk greets patient and determines whether they are an appt. or walk-in. Walk-ins are offered a same day appt. if available (then proceed as an appointed patient). Front desk registers the patient, verifying insurance and demographics. Co-pay collected, if appropriate. Encounter form is printed, and together with ROUTINE TESTING FLOW SHEET, placed in bin for pick-up by Dental Hygienist or Tech. Waiting Room Dental Hygienist or Tech escorts patient to dental operatory. During intake Hygienist/Tech offers RAPID HIV SCREENING. If accepted, Dental Hygienist/Tech obtains sample and transports it for processing (In states where patients are required to ‘opt-in’ to an HIV test, the patient will need to sign a consent form at this time). Dentist performs exam; orders necessary follow-up visits and/or referrals. Hygienist/Tech enters rapid test results on Routine Screening Flow Sheet. “Negative” results given by Hygienist or Dentist; Reactive results given by Dentist. Patient discharged; Hygienist/Tech places the Routine Screening Flow Sheet in project folder. Dental Operatory Laboratory If rapid HIV test is “reactive”; Clinical Coordinator from the primary care center is called to escort patient to the primary care center for a blood draw. Patient takes encounter form and proceeds to check out area for scheduling of next visit. Co-payment collected, if appropriate.
