Loyola University Medical Center Stroke Program Overview • Guidelines for Management of Acute Stroke and TIA – Developed and implemented May 2003, revised June 2005 • 2NE Acute Stroke Unit opened January 2005 • Get With The Guidelines patient management tool usage implemented January 2005 • JCAHO certification as Primary Stroke Center – January 2006 • JCAHO intracycle review approved January 2007

  3. Get With The Guidelines Overview • Developed by the American Stroke Association to improve treatment of acute stroke and prevent future strokes • Allows for collection, maintenance, analysis, and communication of stroke data • Facilitates continuous quality improvement

  4. Project Aim Statement • The patient and/or caregiver for all patients with ischemic stroke, hemorrhagic stroke or TIA receive stroke education and resources

  5. Measurement Goal and Target • Numerator: Number of patients and/or caregivers who received stroke education and resource materials during their hospitalization • Denominator: Number of patients with ischemic stroke, hemorrhagic stroke or TIA

  6. Solutions ImplementedPatient Education Materials • Formal stroke education packets developed and implemented February 2005 • American Stroke Association brochures purchased for patient distribution and placement in the neuroscience waiting room – January 2006

  7. Solutions ImplementedNursing Education • Stroke lecture series offered to nursing staff – November 2005 and 2006 • Stroke Computer Based Learning module nursing competency – November 2005 and 2006 • NIH Stroke Scale training and certification competency neuroscience nurses – February 2007

  8. Solutions Implemented • Ongoing new neurology resident education – July 2005 and 2006 • Neurology Progress Note template with patient education prompts – August 2005 • Implementation of physician EPIC progress notes with education prompts – October 2006

Next Steps • Develop and implement customized stroke Patient Education Nursing Documentation Tool – April 2007 • Develop and implement Nursing Practice Guideline for Utilization of NIH Stroke Scale Assessment • Continue ongoing nursing and physician education related to primary and secondary stroke prevention
