Register for your sport: Set Up a Family Account Things To Remember: • If You Are New To PHS You Will Need Your Child’s Student I.D. to Unlock The System & Create a Family Account. Transfer Students Who Recently Enrolled Need To Register After July 8, 2019. The Final Step In Registering Your First Child Will Require You To Enter A User Name and Password. If you experience problems please use FireFox or Chrome as your web browser. • ONLY CREATE ONE Family Account For All of Your Children at Princeton Public Schools. • Print a blank Physical Form HERE! BP, PULSE, & VISION SCREENING RESULTS are required to be written on your Physical Evaluation Forms!!!Do Not Upload Forms With Blank Sections, Missing Signatures or Missing Physical dates etc. • Physical Forms NEED To BE SCREENED Prior To Uploading Them To Your Child’s Account. • Over The Summer Forms Can Be Scanned At The Scanning Station In The PHS Athletics Office Between The Hours of 8 a.m. and 2 p.m. Completed Forms must be screened and returned to the PHS Athletics Office or Nurses Office at Princeton High school. • Every Athlete Is Required To Take and Pass an Impact Test Every Two Years. Students can register through the PHS Athletics Website.

The Athletic Dept. utilizes voice notification & text message notification. User Name: First Initial followed by Last Name @princetonk12.org (no spaces) Password: Same password that is used to log into any computer at JWMS or Princeton HS

19. Frequently Asked Questions • I forgot my username and/or password? Answer: Use the forgot username/password feature on the login page. 2. I recently enrolled my child at PHS and my student ID is not working. What should I do? Answer: New Students Who Register Before July 4th weekend are unable to register until July 10th. 2. I enrolled my student after July 10th at PHS and my child’s student ID is not allowing me to create a family account. What should I do? Answer: After July 10th please wait 24hrs before attempting to create a family account to register your child for a sport. 3. I am new to Princeton and do not see any ImPact Tests being offered. What should I do? Answer: Send an email to CarlosSalazar@princetonk12.org requesting an impact test.
