Reminders • Please use the current form located in the HR Manager Toolkit/Forms and Documents (NOTE: TBH transactions and nomination forms/ PAFs will not be processed if the current form is not used). • Under Employer Information, always include the department’s physical address (the University’s Mailing Address has been pre-populated). • The employee’s Pay Rate equals semi-monthly rate for Officers and hourly rate for Support Staff (NOT the annual or comp rate). You can use the Pay Frequency Rate from the PAF. • The Pay Rate must include any guaranteed compensation (and should not include any variable or non-guaranteed compensation). • Under the Notice Given section 2, DO NOT use the On or Before February option which is reserved for the annual process ONLY. Always use At Hire for all new hires and Before a change in pay rate (s), allowances claimed for payday for all other changes. • Housing allowances (for Officers) should be listed under section 4 under lodging.
