  5. Working together We accept referrals from any health or care professional working with vulnerable Brent residents: • Hospitals • Social Services • Emergency Services Simple On –line referral form -www.brent.gov.uk/phsreferralform Perpetual Communications Plan to address CHURN

Reviewing the impact of what we do There is a Pilot to more effectively measure what we do looking at, such as: • Impact on customer well-being – STAR survey (before and after adaptations) • Care-package reviews (post adaptation) • Savings we are making from speedier, direct interventions – whole system savings e.g. hospital delays, reduce need for step-down accommodation
