  3. Applications • Employees must re-enroll in Health Insurance • -Choose from Traditional or PPO • -Review PPO Provider Directories and if Provider is NOT on PPO list, decide if PPO is appropriate. Paper • Download and print a PDF of Application on Insurance website • Completed application due May 28 at 5:00 PM to Agency Payroll Office • Agency Payroll Office will enter your information via IPOPS Online • Go to website to complete application online • Agency Payroll Office will enter your information via IPOPS February 25, 2012
