Reasons for Denial… • The FRN references a Form 470 which has not been certified. • During application review, you were asked to demonstrate that when you filed your Form 471 you had secured access to the funds needed to pay your portion of the charges, and you were unable to do so. Reasons for Denial… • A contract for service was signed prior to the due date specified in the RFP. • Applicant has not provided Service Provider Identification Number required to process funding request. • No contract or legally binding agreement was in place when the Form 471 certification was filed. Competitive bidding violation occurs when SP associated with Form 470 participates in competitive bidding process. • This FRN is a request for Telecommunications Service from a carrier that does not provide telecommunications on a common carriage basis.
