  2. What Are HEALTHLINK Services? The HealthLink Program is a collaborative effort between Public Health Nursing, all County programs, Schools and Community Partners. FOCUS: to further the vision of Live Well San Diego by • Working to improve the health, safety, and self-sufficiency of school-aged youth, families, and communities of San Diego County • Supporting healthy choices • Improving systems and policies that support health • Serving as Champions for Changing the Culture From Within

  3. Community HEALTHLINK Nursing Efforts The HealthLink Program is tailored to respond to the unique needs of each region. HealthLink PHNs: • Assess regional needs • Identify priorities and partnership opportunities • Encourage implementation of best practices • Outreach with County programs • Establish and foster authentic relationships with local schools and community partners • Serve as liaison between County and community partners • Help coordinate Grandparents Raising Grandchildren events • Collaborate with Jacobs & Cushman San Diego Food Bank • Coordinate Love Your Heart events
