BUILDING AN EDUCATION PLAN BINDER The life of a parent consists of many challenging and rewarding experiences, especially when faced with a child that has a designation such as a learning disability or special need. The following is a guide to assist parents and caregivers in creating an educational plan binder for your child. This is a great tool as it consists of all up-to-date information and documents of your child’s needs and care. This inventory can be kept in a three-ring binder, and will include the sections as listed on the following slide.

TAB 1.ASSESSMENTS The first tab includes all educational assessments your child has received from professionals. i.e. doctors, etc. The different forms of assessments should be separated into sections. For example: Educational Case Assessments Psych Ed Assessments Learning Assessments Health Assessments Family Doctor/Pediatrician reports Behavioral reports Speech reports Physiotherapist Reports
TAB 2.INDIVIDUAL EDUCATION PLANS (IEP) This section includes any of your child’s Individual Education Plans. An IEP implements modifications in the classroom that assist a student in meeting their educational needs. If you are in the process of developing a Individual Education Plan, this resource will be of great assistance. Please refer to the helpful links section for tools on developing an Individualized Education Plan.
TAB 3.REPORT CARDS This section includes all.. Report cards/interim reports that come from the school regarding your child’s progress. Photocopies of any notes attached to your son/daughter’s report cards.
TAB 4.CORRESPONDENCE TO SCHOOL SYSTEM In this section you may keep a copy of all letters and reports you send to the school regarding your child’s: Progress Needs Behavior
TAB 5.CORRESPONDENCE FROM SCHOOL SYSTEM This tab includes any written correspondence you receive from the school. Written correspondence being any notes or important documents. Keeping these documents in this section will help make them more easily accessible for you.
TAB 6.HEALTH RECORDS The fourth tab includes photocopies of all your child’s important medical records. Important records being any: Testing Medical examinations Diagnostic reports Important health records such as health card Immunizations- your child’s doctor or local health authority will usually provide you with a list of all immunizations including the date received.
TAB 7.DAILY LIVING AND ROUTINE Routine plays a very important part in your child’s life and can be very beneficial in an educational setting. A daily routine will help to make your child’s educational experience more accommodating and comfortable. This section helps indicate if your child has any daily needs. For example: Specialized Transportation Personal support i.e. using the bathroom
TAB 8.CALENDAR This section includes all upcoming appointments along with the dates and times they will take place. Format for appointments may include: Date of appointment _________________ Professional’s name __________________ Type of appointment _________________
TAB 9.MEETINGS This section may include all outcomes of any meetings you have with professionals regarding your child’s progress, needs or behavior. Outcomes being any: Resolutions Results of the meeting A plan you have put into place (i.e. is your child struggling in school? Do adjustments have to be made in the Individual Education Plan?) You may also want to include any notes taken or received during these meetings.
TAB 10.PHONE LOG A phone log is all the conversations you have with a professional on your child’s care. All important information that you receive from the professional may be placed in this tab. Your notes may include: Professional’s name and field (i.e. doctor, speech therapist) Date and time of the call Reason for the call (all important information professional discusses with you) Outcome (is there any action that needs to be taken? A plan you and the professional have put into place?)
TAB 11.CONTACTS For your convenience, you may include a list of all of your child’s educational contacts such as: Occupational therapist Speech pathologist
EMERGENCY CARE PLAN On this website you will find a variety of useful documents including an emergency care plan for children with special needs. Emergencies can occur at any time for families. The key to coping with these unexpected events is to plan. For a family with a child with special needs, it is imperative to put a plan in place. This booklet allows you to have all of your important information in one place. You may want to consider placing a copy of your plan in your binder. This will make it easily accessible for you if an emergency situation is to ever arise.
