Practices & People • Stafford & Surrounds Clinical Commissioning Group (CCG) commissions services for a population of around 145,000 • Twelve GP Practices are within the boundaries of Stafford Borough Council (representing 126,000 patients) • Two GP Practices are within the boundaries of South Staffordshire District Council (representing 19,000 patients) • All 14 GP Practices have two ‘seats’ on the CCG Membership Board to support clinical engagement

National Context • Although the majority of decisions will be made at a local level, CCGs will not work in isolation • There are a number of national policies that will inform and define how CCGs will operate. These include: • The Government’s White Paper, Equity and Excellence: Liberating the NHS (2010) • Putting patients at the heart of everything we do • The Health and Social Care Bill (2012) • Promoting patient choice and competition - No decision about me, without me • The NHS Operating Framework 2012/13 • Ensuring dignified and compassionate care, and strong performance on finance and service quality • Quality, Innovation, Prevention & Productivity

Local Context • Locally SaS CCG aspires to achieve a high quality, clinically safe and financially viable health and social care economy • It will do this through working with its key providers and stakeholders, which include: • Cannock Chase Clinical Commissioning Group • Mid Staffordshire NHS Foundation Trust • Staffordshire and Stoke-on-Trent NHS Partnership Trust • South Staffordshire and Shropshire NHS Foundation Trust • Royal Wolverhampton NHS Trust • University Hospital of North Staffordshire • Staffordshire County Council • Patients and the public • Community and Voluntary Sector

  4. Local Challenges • Mid Staffordshire NHS Foundation Trust and financial sustainability • Clinical Services Implementation Plan (CSIP) including Emergency and Urgent Care Service • Monitor Review • Quality, Innovation, Productivity and Prevention (QIPP)

Health Challenges • Inequalities over life expectancy – on average there is a seven year gap between the ward with highest life expectancy and the ward with the lowest • One ward is in the 10% most deprived wards in England and three are in the top 20% of most deprived wards • High numbers of patients undiagnosed or unrecorded on GP disease registers • Increasing alcohol related and emergency admissions • Performing less than Staffordshire average on Carer’s support, winter deaths and accidental injuries

Goals: By March 2022 Stafford and Surrounds CCG will achieve: • A 10% reduction the levels of obesity against the expected prevalence • A reduction in the proportion of people with undiagnosed disease from 30 – 10 % • A “levelling up” of health outcomes so that all residents experience the same health care outcomes • A reduction in excess winter deaths of 50% • A reduction in unplanned admissions to hospital for people with Long Term Conditions of 50%

  8. We will deliver our goals through: • Clinically led processes and structures • Good engagement with patients, the public and our stakeholders • Clear commissioning intentions • Collaborative commissioning

Membership Scheme Stafford & Surrounds CCG Executive Board (Including PPI Lay Member and representatives from Public Health, & Borough Council District Patient Participation Group (Including representation from all 14 practice PPGs, PPI Executive Lead, HealthWatch & CCG representatives) Thematic Groups/Pressure Groups Practice Based Patient Participation Groups (Attached to all 14 GP Practices and representing the 145,000 residents of Stafford and surrounding villages)

  10. We will measure success through: • O – Organisation Outcomes • S – Satisfaction Outcomes • C – Clinical Outcomes • A – Activity Outcomes • R – Resource Utilisation Outcomes
