  4. Definities • Dyspneu = klacht van de patiënt • Respiratoire distress = waarneming van de toeschouwer • Respiratoire insufficiëntie = stoornis in de arteriële bloedgassen

  5. Pathofysiologie • Ontstaat in seconden tot uren. • Toegenomen ventilatie door centrale prikkeling of perifere stimulatie. • Uitzonderingen? • Bij neuromusculaire aandoeningen kan de ventilatie weinig of niet verhogen. • Centrale stimulatie wordt vaak niet als dyspneu ervaren.

  6. Pathofysiologie • Centrale stimulatie: - aandoeningen CZS - acidose - psychogeen

  7. Pathofysiologie • Perifere stimulatie - verminderde aanvoer - toegenomen verbruik

  8. Differentiaal Diagnose • Denk per systeem - CZS - Neuromusculair - Thorax wand - Longen - Cardiovasculair - Hogere luchtwegen - Overige

  9. Tweede benadering Lichamelijk onderzoek: A: Inspectie mondholte Vreemd voorwerp verwijderen Epiglottitis → AB iv Glottisoedeem → Adrenaline en cortico’s

  10. Tweede benadering B: Auscultatie: Longoedeem → Diuretica Infecties → AB iv Hypersonore percussie: Pneumothorax → Drain

  11. B: Gestoorde thoraxmobiliteit: Intuberen bij een fladderthorax Hyperinflatie unilateraal: Pneumothorax → Drain Hyperinflatie Bilateraal: Bronchusobstructie → Dilateren Hyperventilatie: Rebreathing

  12. Tweede benadering C: Pulsus paradoxus: Extreme bronchusobstructie → Dilateren Pericardtamponade → Punctie Verhoogde CVD Pericardtamponade Longembolie

  13. Behandeling/ in de dokterstas • O2 • Handschoenen • Adrenaline en cortico’s • Diuretica • Zakje • Bronchusdilatatoren (voorzetkamer en iv)

  14. Take home-message • Een groot deel van de behandeling van acute dyspneu vindt plaats in het ZH • Het belangrijkste gebeurt echter bij het eerste contact waar de A, B en C kunnen worden veiliggesteld. • Zorg dus dat je je jouw spullen bij je hebt!
