Who We Are • Our aim:to improve the health status of children and adolescents in San Francisco by promoting healthy eating and physical activity. • Our commitment:to work with communities which are the most impacted by environmental barriers and to support families in creating and sustaining healthy practices. School-Based • Our vision:to extend beyond traditional disease education and to change schools themselves, integrating healthy practices into the school environment so that students experience health as well as learn about it. • Our goal:to permeate each school day with opportunities for nutrition and physical activity and to establish the teacher buy-in and policy support to maintain these activities. School-Based • Program Description: Our school-based program has worked with over 3000 families, students, and school staff to increase physical activity and healthy eating opportunities in San Francisco Schools since 1999. • School Communities: Lawton Alternative School (K-8) 1999 to 2003 Rooftop Alternative School (K-8) 1999 to 2003 De Marillac Academy (4-8) 2004 to present McKinley Elementary School (K-5) 2004 to present Shape Up SF Program Snapshot
MOVE • Pediatric Weight Management Program • Combine best practices in pediatric weight management to address overweight. • Our goals are to work with children and families to achieve weight maintenance or weight loss and to develop healthier lifestyles. Shape Up SF Program Snapshot
MOVE Clinic • Clinic Description: • Multidisciplinary team of medical professionals • Full medical assessment and follow-up appointments • When:Mondays 8-12pm & 4-6pm • Who:2-18 years olds with a BMI at or above the 85th percentile for age on the 2000 CDC growth charts • Where:CPMC Pediatric Specialty Clinic 3700 California St. B555 Shape Up SF Program Snapshot
MOVE Classes • Class Description: • 13 week family group classes • Taught by behavior coaches • Who: 8-13 years olds with a BMI at or above the 85th percentile for age on the 2000 CDC growth charts and their families • Where: Bayview Child Health Center 1335 Evans Street • When: Tuesdays, 4-6pm Shape Up SF Program Snapshot
