  3. Cause of Accident • Anchor let Loose During Anchoring Ops Causing it to Hit Against the Hull Beneath the Water Line.

  7. Investigation Reveals • Worn cat’s paw loosened during anchor ops, failed then allowed anchor to drop. They hit the brakes making the anchor swing into the hull causing a crack. • Final soundings had 97 gallons unaccounted for • Wind and current heading away from shore (minimal impact)

  8. End Results • Minimal Damage to the Environment • Hull Repairs were Made at the Liberty Terminal • NOV was Issued
