This guide provides an overview of the responsibilities of different members in the transition planning process for students with disabilities. It helps students and families navigate the transition process, identify participants and their responsibilities, and understand expectations based on their role in the IEP team.

Purpose This document is intended to help students with disabilities and families: • Navigate the transition process • Identify participants in the transition planning process • Identify participants’ responsibilities • Identify expectations of others based on your role in the IEP team • Identify possible resources

  4. Considerations • Expectations are broad expectations. • Your local school district may have additional expectations • Check in with your child’s school for additional expectations • Feel free to add local expectations within the notes section

  5. Acronyms to Consider: • IEP = individualized education program • IPE = individualized plan for employment • ISP = individualized service plan • PCP = person-centered plans • VR = vocational rehabilitation • DHS = Department of Human Services • SoP = Summary of Performance • DD = developmental disability
