Benefits of Group Help and support Sounding-board Share information about suppliers Share information about innovative products Share innovations around service re-design For future… West Yorkshire purchasing group Expanding group to the North East We offer help and support to each other – particularly when we were all setting up our services. Shared information around costs, referral processes, assessments etc. We act as a sounding board for one another. Share information about products and suppliers. Share ideas about service re-design. We also act as a forum for feedback to suppliers and somewhere for suppliers to come and show new equipment. In future we hope to put together a purchasing group to gain better deals using PASA framework and we have had a lot of interest from other areas in the North East so we are discussing ways of expanding. Any Project leads interested who haven’t already contacted me please do so today. We will be having a large meeting in Feb/March time to take things forward.We offer help and support to each other – particularly when we were all setting up our services. West Yorkshire Telecare Services All now have Telecare Services in place All will continue after PTG finishes All know about one another’s services All came from different starting points e.g. Wakefield had some Telecare already in place through Wakefield District Housing, others did not. Calderdale had a mobile Response service for callers. We also outsource many aspects of our service to private companies. We’re all different.     8. Case Study Lady with Dementia Family and neighbour concerned about night-time wandering. Lady not aware – thought she stayed in bed Installed Just Checking system Established clear picture of activity Working with Community Mental Health team to maintain her independence A lady with dementia had caused concern for the social worker and the family when the neighbour complained about the level of night time activity. The service user was not aware of this activity and thought that she spent the night in bed. They installed the Just Checking system. Series of sensor’s which collate information about how a person is moving around their home and send that to a website. The social worker and family were able to establish a clear picture of the level of night time activity. They then brought in a community mental health worker to work with the lady and establish the best way of maintaining the lady’s independence in her own home. There are other systems on the market such as Quiet Care that do a similar thing.A lady with dementia had caused concern for the social worker and the family when the neighbour complained about the level of night time activity. Calderdale Launched service in November 2006 Around 200 Telecare solutions Telecare - Falls strategy Doing LTC pilot - Telehealth By Telecare solutions I mean not including Social alarms or smoke detectors. Big focus around Falls due to successful POPPs bid – made Telecare an integral part of that strategy.     10. Case Study New Falls Strategy (POPPs) Falls Prevention Workers Install TeleMedCare Triax Technology in Falls Clinic Launched pathway at joint event with Kirklees. Focus is on finding those people who have fallen or who are at risk of falling and getting them help asap. Have employed 4 Falls prevention workers who go and visit people in their own homes and one of things they can offer is telecare where appropriate – will fit. Technology given to those who present without a cause to falls clinic – fitted up to 2 weeks before they attend clinic. Gathers information about their movements; professionals log on and look at data; also perform all sit stand tests in clinic wearing the device. Launched pathway at joint event with Kirklees. Case Study Elderly lady with Downs Syndrome Early signs of Dementia Receives domiciliary support Started wandering Installed Tunstall Property Exit Sensor Link to NKDSS who then can accompany her Can assess any deterioration Elderly lady with Downs Syndrome and early signs of Dementia. She lives semi-independently in a flat along with a flat mate, also an elderly lady with Downs Syndrome. They receive domiciliary support from North Kirklees Domiciliary Support Service. She recently started wandering and her dementia is showing signs of deterioration and her walking has become an increasing risk. Installed Property Exit sensors which link to NKDSS. When she goes for a walk the system automatically lets NKDSS know so they can follow or accompany her. This gives the staff more specific information about her pattern of walking helping medical staff to assess any deterioration. Difference – given her the choice to continue to live in her flat with her flat mate. Kept her life to her routines and prevented fast deterioration.     13. Leeds Service launched in October 2006 1308 homes have had Telecare installations to date 1800 referrals including 1100 smoke detectors

    14. Case Study COPD Pilot with Leeds NHS Trust Tunstall Genesis Units installed Monitored by COPD nurse Agreement to support other LTC’s Have done a small COPD Pilot with Leeds NHS Trust. Used Tunstall’s Genesis Units which were installed in service user’s homes. The users themselves then took a variety of measurements on a regular basis and the information was downloaded to a website which was monitored by a COPD nurse. The pilot has bee so successful that they are now expanding it and looking at the possibility of monitoring other Long Term Conditions in this way. Have done a small COPD Pilot with Leeds NHS Trust. Case Study Young woman with Cerebral Palsy and learning difficulties Difficulty swallowing – only mashed food Risk – gaining access to kitchen Installed Chubb Carer Alert and Care Sensor Young woman lives in supported housing for learning difficulties with 3 other ladies. Has cerebral palsy and learning difficulties. Has difficulty swallowing and can only eat forked, mashed food. Likes to pour water out of the hot kettle and has unpredictable behaviour. Had been admitted to hospital previously related to these behaviours. Risk was around gaining access to the kitchen. A Chubb Carer Alert and Care sensor were installed along with door contacts to the kitchen. Staff were aware as soon as she got out of bed at night – stopped her getting into the kitchen and eating the wrong types of food and then choking and being admitted to hospital.
