§R325.99408(4) “A person or individual who conducts a lead-based paint investigation shall notify the department, on forms provided by the department, regarding information the department considers necessary, by the fifteenth day of the month following the lead-based paint investigation.”
Form DCH-0694-e Submit by 15th day of month following activity E-mail to linced@michigan.gov Subject line should include: “LBP ID Report” file name format: “monthyear_P####”. Example: October 2007 report for person with P-9999 would name the file “1007_P9999.xls”.
