Important Dates • Student Graduates • Must graduate by 8/31/17 or receive GED by 8/31/17
Dropout Recovery • No show students • Use the White/Grey No Show Leaver Form • Students who are in attendance in the 2016-17 School year for at least one full reporting day • Use White Request for Withdrawal
MOST IMPORTANT • We do not coerce students into Homeschooling • Do not alter the Leaver Forms • Leaver Forms MUST be complete with dates and signatures • We MUST verify enrollment • We MUST not create documentation • Call Tyler with any questions at any time
Documentation for No-Shows • Must be currently enrolled • Must have date and signature • Must have documentation from Parents, New school or Qualified student • Student must be on PET before their leaver code may be changed
Summer Information • Begin to change the leaver codes for students that a campus has received information AFTER ROLLOVER. • Ensure the No-show Leaver form is properly completed and has the leaver documentation attached.
Current Year Withdrawals • Must have an exit interview conducted by Counselor, Assistant Principal or Principal • All dates and signature lines complete before filing • May only have one leaver code (Example-cannot home school for GED) • Always ask for additional information in writing
Campus Audits- Issues • RFW missing Authorized Representative signatures on front and back • Exit interviews being conducted by support staff; later signed by the Administrator • Missing parent signatures; • Incomplete supporting documentation regarding conversations
Withdrawal Forms • Must use the RFW Forms; • White with Watermark • Includes new leaver codes • Parents are only allowed to complete Demographic section • Must order from Print Services; No photocopies or alterations
Qualified Student • Any student who one or more of the below: • Married • is 18 years or older, or • has established a residence separate and apart from the student’s parent, guardian, or other person having lawful control of the student
Qualified Students • May sign the RFW • May indicate that they will Home School “60” themselves; Please have them indicate if they are enrolling in an online program and information pertaining to the program.
Parents/Guardians not present at time of withdrawal • The Authorized Representative shall fill out the District Request for Withdrawal form • Identify by name, the person requesting the withdrawal. Staff must confirm that the person requesting the withdrawal is a Parent/Guardian/Responsible adult (POA or Affidavit of Responsibility) or a “qualified student” • The type of contact (phone, email, etc.) by the individual requesting the withdrawal
Parent/Guardian not present… • Date of contact • Content of the conversation (answer the who, what, when, where, why questions) • If the request for withdrawal is made by email, fax, mail and/or any other type of written form, the document must be attached to the District Request for Withdrawal form • Notation of the campus personnel taking the required information • Obtain Authorized School District’s Representative’s signature • Student must be currently enrolled
Current Leaver Codes • 16- Returning to Home Country • Can have info from any adult with knowledge • Does not have to be born in that country • Use for Exchange Students
(03) Student Died • Documentation- brochure from funeral, obituary or death certificate
(24)Enrolled in College • Used for students who have not graduated and are attending college • Must have been or currently enrolled in 9 Credit hours • Documentation includes: Transcript, verbal confirmation with college registrar and/ or receipt of payment for college hours.
87 Enrolled in Tx Tech or UT Online • School must have records request for the student information • CODE IS LOCKED. MUST SEEK APPROVAL FROM DROPOUT COORDINATOR
(81) Private School • MUST BE CURRENTLY ENROLLED • Student/ Parents must give school name • Give the parents the list of options with the disclaimer if they ask for your opinion. • Audits check for high use of code and school destination • Must have verification of enrollment documented from phone call or in writing. • Records Request is the best form of documentation
(82)Moving out of State • MUST BE CURRENTLY ENROLLED • Parent/ Student must give specific destination. • Ask which school they will attend • Ask parent/student to have new school send records request • Must have verification of enrollment documented from phone call or in writing. • Records Request is the best form of documentation
(60) Home School • MUST BE CURRENTLY ENROLLED • Parents must instigate the desire to home school their child • Ask parent/ qualified student to provide a hand written or self-typed Letter of Assurance that includes: • Start date • Signature • Qualified student can home school themselves • Cannot Home School for GED (98)
(86)Student Completed a GED outside of Texas • Copy of the GED certificate
(85) Graduated Outside of Texas • Graduated prior to entering school and then left • Documentation • Transcript with sufficient credits
(66) CPS Removal • Must have documentation from CPS, not the foster caregiver • Documentation must include • CPS Official information • Removal Paperwork
(83) Administrative Withdrawal • Student never lived within the schools boundaries and provided a false address • Not to be used for truancy or attendance • Student does not have current state mandated inoculations • This code must be approved through District Dropout Coordinator prior to use.
(88) Court Ordered GED • Must have a copy of the Court Order • Has name of the student, date of the order, name of the judge making the order and county the judge presides in • Must be Rightfaxed and approved by Dropout Coordinator • CODE IS LOCKED. MUST SEEK APPROVAL FROM DROPOUT COORDINATOR
(89) Incarcerated as an Adult • To be used for state jail or federal penitentiary • Student must be an adult or certified as an adult • Documentation • Written documentation of an oral notice from Law Enforcement • Written notice from Law Enforcement • CODE IS LOCKED. MUST SEEK APPROVAL FROM DROPOUT COORDINATOR
(90) Interstate Compact • Student has an active duty military parent, then transferred to Texas Pub. School during senior year. • Can not graduate with TX requirements but will meet sending state requirements • Under compact, student could graduate • Documentation • Transcript with sufficient credits • Diploma with graduation seal
Authorized School District Representative Who is an “Authorized School District Representative”? The principal and those individuals he/she identifies each year using the following form: ANNUAL CAMPUS DESIGNATION OF RESPONSIBILITY FOR SCHOOL LEAVER PROCESSING Special Note: • This form is an auditable form and completed at the beginning of each school year. A copy is sent to the Dropout Coordinator and kept on file. • Representatives are limited to the Principal, Administrators, Counselors.
DAEP Student Referrals • Monitor DP report ( STJ0266D) • sent via email identifying students recommended and placed at DAEP who have not shown up after 3 days • follow up with phone calls to the family to avoid a drop out situation.
Stamp • Ink stamp Compulsory Attendance Warning • Still available for order • To be used on the Mainframe Withdrawal that students/ parents take with them.
Expected Results • Campus dropout coordinators will monitor the withdrawal documentation • District dropout coordinator will work closely with campuses and monitor all leaver codes, dropout code 98, and the documentation to support the code
Expected Results continued • Principals and campus dropout coordinators will be responsible for providing yearly training for staff involved in the withdrawal procedure • Principals and campus dropout coordinators will be responsible for provide training for any new campus personnel hired during the school year
