INTRODUCTION & BACK GROUND FUT is still the "GOLD STANDARD " to me and it is very much needed to evaluate the reasons of decline in FUT technique all over the world.The most of the clinics all around the world have adopted FUE technique as the only procedure inspite of the fact that it has more labour work for the Hair Transplant Surgeon and more expensive for the patients. Why FUE technique became more popular in the Patients and Doctors? Why FUE became the major & only demand of the patients while in consultation with Hair Restoration Surgeons .I think three main reasons pushed FUT technique down among the patients & Physicians. 1.. Post operative pain. 2.. wide prominent linear scars at donor sites. 3.. Less number of grafts and eventually poor results to meet the patients needs. As FUT technique is bouncing back strongly in my after continuous efforts after losing its popularity because of many issues in FUE procedure and technique ,It is need of the hour to investigate the weak sides of the FUT procedure and consider most possible solutions to upgrade the procedure . Before going towards the solutions its important to highlight the weak areas. The issues coming to us as a Hair Transplant surgeon were through questions and complaints in pre operative and post operative meetings with the patients like most of the patients were not satisfied with the intensity of pain ,wide post operative linear scars at the donor areas ,Tension and permanent Numbness around and above donor sites and more over less number of grafts ..The patients are heard saying that instead having this much severe pain, wide linear scar and permanent Numbness we got no improvement...... We as a Hair Transplant surgeons should take all the steps to improve all these steps ... I have introduced the Cocktail of steps to get improvements in all slots ....like combination of Skin separation ,undermining both edges and Trichophytic double layered closure gave me a huge comfort in all areas mentioned above

  3. OBJECTIVES •1.. Reduce the postoperative pain. •2.. Improve the scars at the donor sites. •3.. We can increase number of the grafts in each surgery.

  5. FLEXIBILITY/ LAXITY EVALUATION STEP – 1 The Flexibility / Laxity of the donor areas is evaluated by 2 thumb rule . Both thumbs are placed 1 to 1.5 mm apart depending on the width of the strip need to be harvested .Then they are pushed in opposite direction to measure the laxity of the skin .   6. SCORINGS STEP – 2 1/2mm deep scoring is done on both edges with tip of no 10 blade just to engage the blade belly of no 15 blade used for strip separations . An extra scoring is done at the lower edge for trichophytic strip just 1mm ( one Follicular unit ) away from the first scoring line .   7. STRIP SEPARATION STEP – 3 Strip separation is done by sliding the blade belly of No. 15 blade inwards and medically alongside the hair shaft angle at lower edge ,outwards and medially at upper edge. Video Demonstration Click on the link to watch youtube video:

  8. UNDERMINING STEP – 4 Undermining is done on both edges just above the galea and underneath the dermal papilla. Its should be done about 0.5 to 0.7 cms on both edges upper and lower depending upon the requirements . Video Demonstration Click on the link to watch youtube video:

  9. Trichophytic STRIP TRIMMING STEP – 5 1 mm strip at lower edge is trimmed for Trichophytic closures Video Demonstration Click on the link to watch youtube video:

  10. DOUBLE LAYERED CLOSURE STEP – 6 Double layered closure is done to divide the pressure / stretch at suture site. One bite is taken just underneath the dermal papilla and other in upper 1/3rd of the length of Hair Follicle with same thread. Video Demonstration Click on the link to watch youtube video:

  11. DISCUSSION We will discuss all three steps here . Skin separation was done with number 15 blade by sliding the blade belly on the shaft of the Hair Follicle by pushing the blade down and inwards at lower edge and downwards and outwards at the upper edge. We achieved very low transaction of the follicles almost near to the zero and created a wedge at the upper edge to get it fit on lower lip during closure process . Undermining both the edges 05cms to 0.7cms and total undermining on both edges will become more than 1cm. This undermining of more than 1cm will help to restore the laxity to good extent and will reduce the tension at closure site eventually helps to produce thin scars at donor sites. Double layered closure always helps to reduce the tension at suture sites because the main tension is absorbed at the lower layer and upper lip in the wound faces less tension. This ultimately reduces the scar sizes.

  12. CONCLUSION Total number of patients were picked up randomly out of bulk done with these improvements.. By scar more than 1 mm I means in some cases it was 1.1 to 2mm or even more ..... Only 2/3 grafts were seen transacted in whole strip harvested ... I also want to present the video of the procedure and sending it separately for acceptance.
