Business in Medicine ProjectARTAK LABADZHYAN, MD (PGY2)1/27/12 CT Head Orders for 19 Patients Hospitalized on Tele Floor at VA Hospital. Analysis for Unnecessary orders.
My Definitions • Unnecessary repeat test defined as recent CT (either 1-7 days prior; or within last 2-4 months); for the same indication; with different imaging recommendation on previous CT; or no new indication. • Blatant: repeated in last 2-3 days with no new indication/event
CONCLUSION • 19 pts on the telemetry unit were analized. 8 pts overall received a CT of the head, with 5 total having CT of head repeated either within last 4 months or within last 1-7 days. • 3 of the tests were unnecessary, 1 of which was blatantly unnecessary. • ED staff was more guilty of unnecessary orders as apposed to TEAM, although one blatant fault was by TEAM • Overall, the reasons for unnecessarily re-ordering CT of head was neglecting to check if same study was ordered within 1week, or within 4 months. Neglecting to check prior indication to current indication. Neglecting to order new imaging recommended from prior CT (i.e. MRI recommended).
