Influenza Case Summary • Aggregate reporting of lab-confirmed influenza cases. • Data shall be aggregately collected and reported for a one week period (runs Sunday to Saturday). • Reporters complete excel spreadsheet to collect cases by condition category and age group by jurisdiction (local health agency). • Every Monday reports are emailed or faxed to the appropriate jurisdiction (local health agency). • Jurisdictions enter data onto excel spreadsheets, then transfer data into Aggregate Reporting - Influenza Case Summary application.
Enter search criteria (at least one field) Search dates do not have to be a Sunday or Saturday The jurisdiction is restricted to the jurisdiction(s) the user has access to. Click Search
Search Results Screen If you need to edit a report that has already been entered, click Edit.
Update Case Summary Screen The only item that can be edited is the Number of Cases. Click Save
Animal Bite Weekly Counts • Report Date: Date is to represent the CDC week (runs Sunday to Saturday). Only a Sunday date can be entered. • Jurisdiction: Select the jurisdiction you are entering for, such as Cole County or Independence. • Animal: Select the animal you are entering the count for. • Number of Cases: Enter number of cases reflective of the animal type for the report week in the jurisdiction.

Update Case Count Screen The only items that can be edited are the Animal Type (update only if the type is missing) and the number of bites. Click Save
Reporting WebSurv Problems • If you are having problems with WebSurv call the appropriate persons for assistance: • For validation errors (i.e., forget to add in a required field etc.) and WebSurv is still on your screen: Contact your district DHSS contact (see Contact Information slides) • For Runtime Errors: Screen changes and there is a message that says runtime error and other “IT” stuff: Contact the ITSD Help Desk by emailing support@dhss.mo.gov or by calling 800-347-0887
Crystal Reports • Query’s developed for quick availability to data in WebSurv • It is important to regularly check open records in Crystal Reports • Internet link: http://crystalweb.dhss.mo.gov/businessobjects/enterprise115/InfoView/logon.aspx • Intranet link: http://crystal1/businessobjects/enterprise115/InfoView/logon.aspx • The user ID is the same as for WebSurv and the password is your first & last initials and the last four digits of your social security number.
A directory of the reports can be found at http://10.33.34.5/mohsaic/ under Crystal Reports, Report Catalogs, then WebSurv Reports.
Future Rollouts • In addition to existing functionality of WebSurv future enhancements to the system are currently being developed. • Additional disease specific supplemental screens • As new disease specific supplemental screens are added the existing paper forms will be updated for consistency. • Enhancements to the system identified during the initial implementation of the system.
