COMUNICADO • Por disposición de la Oficina General de Administración del Ministerio de Saludy con la finalidad de agilizar el trámite, la solicitud por Devolución presentada en Trámite Documentariodel MINSA, deberán consignar obligatoriamente los siguientes datos: • Número de D.N.I. o R.U.C. según corresponda. • Número Telefónico. • Número de cuenta de ahorros ó Código de Cuenta Interbancaria (CCI). • Presentar voucher o recibo Adquiriente o Usuario y Expediente original. • En caso de pérdida o robo presentar Denuncia original. • Si hubo un error en la consignación de número R.U.C., deberá presentar la solicitud de rectificación respectiva acompañada de la carta de autorización emitida por la empresa titular del R.U.C. • El plazo para solicitar la devolución es de 30 días de efectuado el depósito. • La carta debe ser firmada por el Representante Legal inscrito en la SUNAT. • Atentamente, • Nota: los gastos bancarios correrá por cuenta del solicitante
