Elder Center & Assisted Living Facility-A Continuum of Elder Care Elder Center-Day Services Assisted Living & Memory Care Residential Services
Adult Day Services Nutrition Program Congregate and Home Delivered Personal Care Services Day Services Activities, Exercise, Recreation & Field Trips Veteran Services Home Services & Home Chores Regulatory Monitoring and Training Transportation, Client Assessment, Referrals, and Service Tracking Benefits Coordination and Assistance
Transportation Program • Adult Day Services – To and From Home (daily) • Shopping: groceries, household/personal needs, warehouse pick up for commodities, etc. • Non-Medical: congregate meals, family visiting, traditional support days, outings, special meetings, salon & barber visits, library, access to other tribal program resources; housing, public works, census, social services, tribal courts, governor’s office, etc. • Recreation: • Sr. Olympics – State wide travel • 1 trip per month w/in 50 mile radius of center • 2 overnight trips within state annually • 1 travel outside of state annually
POI Elder Center Fleet – 14 Vehicles Transit and handicap access vans, utility truck and SUV Meal trucks and passenger cars 36 Passenger Shuttle
ESSENTIAL RESPONSIBILITIES FOR TRANSPORTATION PROGRAM MANAGEMENT • Internal Management Systems: • Compliance & Reporting Transportation Scheduling Record Keeping & Reporting • Mileage logs • Passenger data • Vehicle inspection check lists • Maintenance/repair documents • Passenger safety/incident reports • Budgeting and tracking associated costs • Compliance Monitoring and Auditing • Systems to Address Reoccurring Tasks; • Maintenance Schedule • Staff training and Certification • Acquisition/Replacement of Vehicles • Forecast the need and justification • Coordinate the Timing of Resources (local, state and federal)
ESSENTIAL RESPONSIBILITIES FOR TRANSPORTATION PROGRAM MANAGEMENT-Continued • CLIENT SAFETY AND SATISFACTION • Cognizant of Passenger Safety and Sensitivity • Client Satisfaction / Grievance Procedures • Wheelchair Lift Operations • Walking Device Storage (tie down) • Schedule for Driver Certification or Recertification
Sources of Funding for Vehicles • State of New Mexico, Aging and Long Term Services Capital Outlay – Annually • Application Process • Infrastructure Capital Improvement Plan • Application Inclusive of: • Justification/Emphasis of Need • Transportation Service Plan and Service Area • Asset Inventory (age of vehicle, acquisition, mileage, etc.) • Quarterly Vehicle Inspection Requirement
